Description
Overview of the medicine
Fosfomycin is a broad-spectrum antibiotic used to treat acute uncomplicated urinary tract infections (UTIs) caused by susceptible bacteria. It is typically administered as a single oral dose.
Uses & Indications
Dosage
Adults
One sachet (3 gm of fosfomycin) dissolved in 90-120 mL of water and taken as a single oral dose, preferably on an empty stomach or 2-3 hours after food.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function. Use with caution in elderly patients with impaired renal function.
Renal_impairment
Contraindicated in patients with severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Dissolve the entire contents of the sachet in 90-120 mL (approximately half a glass) of water. Stir to dissolve and drink immediately. Do not use hot water.
Mechanism of Action
Inhibits an early stage of bacterial cell wall synthesis by inactivating the enzyme UDP-N-acetylglucosamine enolpyruvyl transferase (MurA).
Pharmacokinetics
Onset
Within 2-4 hours of administration.
Excretion
Primarily excreted unchanged in the urine (around 50%) and feces (around 18%) within 72 hours.
Half life
Plasma half-life is approximately 4-8 hours; urinary half-life is longer.
Absorption
Rapidly absorbed after oral administration, converted to fosfomycin acid. Peak plasma concentrations reached within 2-4 hours. Bioavailability is about 37%.
Metabolism
Not extensively metabolized in the body.
Side Effects
Contraindications
- Known hypersensitivity to fosfomycin or any component of the formulation.
- Severe renal impairment (creatinine clearance <10 mL/min) or patients undergoing hemodialysis.
Drug Interactions
Metoclopramide
Concomitant administration of metoclopramide reduces the peak plasma concentration and urinary excretion of fosfomycin. Avoid co-administration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include diarrhea, abdominal discomfort. In case of overdose, supportive care and symptomatic treatment should be administered. Hemodialysis is not effective.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Fosfomycin is excreted in human milk; use with caution in nursing mothers.
