Description
Overview of the medicine
Fructose 10% w/v Injection is a sterile solution of fructose in water for intravenous administration. It is primarily used as a source of calories and fluid, particularly when glucose metabolism is impaired or when a slower rate of glucose utilization is desired.
Uses & Indications
Dosage
Adults
Dosage varies based on caloric and fluid requirements, typically administered as a continuous intravenous infusion. Max rate usually 0.5 g/kg/hour.
Elderly
Similar to adult dosage, but caution with cardiovascular and renal function.
Renal_impairment
Use with caution and monitor fluid and electrolyte balance; dosage adjustment might be needed in severe impairment.
How to Take
For intravenous infusion only. Administer slowly via a central or large peripheral vein to minimize venous irritation and metabolic disturbances. Do not administer intramuscularly or subcutaneously.
Mechanism of Action
Fructose is a monosaccharide that serves as an energy source. Unlike glucose, its initial metabolism in the liver does not directly require insulin, making it potentially useful in conditions of insulin resistance or impaired glucose tolerance. It is converted to glucose, lactate, or fatty acids for energy or storage.
Pharmacokinetics
Onset
Immediate upon intravenous administration.
Excretion
Metabolites are excreted renally. Unmetabolized fructose is negligibly excreted in urine under normal conditions.
Half life
Plasma half-life is short, typically minutes, as it is rapidly metabolized.
Absorption
Rapidly absorbed into cells after intravenous administration.
Metabolism
Primarily metabolized in the liver via the fructokinase pathway, leading to the formation of fructose-1-phosphate, which is then cleaved into dihydroxyacetone phosphate and glyceraldehyde, eventually entering glycolysis or gluconeogenesis.
Side Effects
Contraindications
- •Hereditary fructose intolerance (HFI)
- •Fructose-1,6-bisphosphatase deficiency
- •Severe metabolic acidosis
- •Known hypersensitivity to fructose
Drug Interactions
Insulin
May reduce the need for exogenous insulin in diabetic patients, though direct insulin stimulation is absent.
Diuretics (e.g., Furosemide)
May enhance fluid and electrolyte imbalance if not monitored.
Drugs affecting uric acid metabolism
Rapid fructose infusion can transiently increase uric acid levels.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Symptoms include metabolic acidosis, hyperuricemia, lactic acidosis, fluid and electrolyte disturbances, hyperglycemia. Management involves stopping the infusion, correcting fluid and electrolyte imbalances, and treating metabolic acidosis.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and potential benefits outweigh risks. Monitor maternal and fetal/infant metabolic status.
