Description
Overview of the medicine
Fluconazole is an antifungal medication used to treat various fungal infections, including vaginal yeast infections, oral thrush, esophageal candidiasis, and systemic candidiasis. It belongs to the azole class of antifungals.
Uses & Indications
Dosage
Adults
Vaginal candidiasis: 150 mg as a single oral dose. Oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. Systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily. Dosage varies greatly by indication.
Elderly
No dosage adjustment needed unless renal function is impaired. Use with caution.
Renal_impairment
Dosage should be reduced based on creatinine clearance (e.g., reduce by 50% for CrCl <50 mL/min).
How to Take
Funga 50 mg capsule can be taken orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14α-demethylation, which is an essential step in fungal ergosterol biosynthesis. This inhibition leads to the accumulation of 14α-methyl sterols and a subsequent decrease in ergosterol, thereby impairing fungal cell membrane integrity and function.
Pharmacokinetics
Onset
Within hours for symptomatic relief, full effect over days.
Excretion
Mainly renal; approximately 80% of the dose is excreted unchanged in the urine.
Half life
Approximately 30 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally; bioavailability is >90% of IV dose. Peak plasma concentrations are reached within 0.5-1.5 hours.
Metabolism
Minimally metabolized; primarily unchanged drug excreted.
Side Effects
Contraindications
- Hypersensitivity to fluconazole, other azole antifungals, or any component of the formulation.
- Co-administration with terfenadine in patients receiving multiple doses of fluconazole 400 mg/day or higher.
- Co-administration with other drugs known to prolong the QT interval and metabolized by CYP3A4 (e.g., cisapride, astemizole, pimozide, quinidine, erythromycin).
Drug Interactions
Warfarin
Increased prothrombin time; monitor INR.
Phenytoin
Increased phenytoin levels; monitor levels.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants; monitor levels.
Sulfonylureas (e.g., glipizide)
Increased hypoglycemic effect; monitor blood glucose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment should be symptomatic and supportive, possibly including gastric lavage and hemodialysis (which significantly reduces plasma levels).
Pregnancy & Lactation
Pregnancy Category D (for long-term or high-dose use); generally avoided unless potential benefits outweigh risks. Consult a doctor. Fluconazole is secreted in breast milk; caution advised during lactation.

Lab Monitoring
- Liver function tests (ALT, AST, bilirubin) in patients with pre-existing liver conditions or on prolonged therapy.
- Renal function in patients with renal impairment.
- Potassium levels if on diuretics or with cardiac conditions.
Doctor Notes
- Fluconazole is a potent CYP2C9 and moderate CYP2C19/3A4 inhibitor; consider potential drug interactions.
- Monitor liver function tests periodically, especially in patients with pre-existing hepatic impairment or during prolonged treatment.
- Exercise caution in patients with proarrhythmic conditions or those on other QT-prolonging agents.
Patient Guidelines
- Complete the full course of medication even if symptoms improve to prevent recurrence.
- Inform your doctor about all other medications you are taking, including herbal supplements.
- Report any severe side effects, especially yellowing of skin/eyes, dark urine, or unusual tiredness.
- Avoid alcohol during treatment, especially with concurrent liver issues.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.
Driving Precautions
Funga 50 mg capsule is generally not expected to affect the ability to drive or operate machinery. However, if you experience dizziness or seizures, avoid these activities.
Lifestyle Advice
- Maintain good hygiene to prevent fungal infections.
