Description
Overview of the medicine
Furoclav 250 mg tablet is a broad-spectrum antibiotic used to treat various bacterial infections. It contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor, which helps to overcome antibiotic resistance.
Uses & Indications
Dosage
Adults
Typically 250 mg to 500 mg twice daily, depending on the severity and type of infection. For most infections, 250 mg twice daily for 7-10 days.
Elderly
Similar to adult dosage, but renal function should be considered and dosage adjusted if necessary.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 10-29 mL/min, 250 mg once daily. For CrCl <10 mL/min, 250 mg every 48 hours.
How to Take
Take orally, preferably with food, to enhance absorption and reduce gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Cefuroxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis. Clavulanic Acid inactivates a wide range of beta-lactamase enzymes produced by bacteria, which would otherwise degrade cefuroxime, thus extending its spectrum of activity.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 1-2 hours after oral administration.
Excretion
Cefuroxime is excreted unchanged by glomerular filtration and tubular secretion. Clavulanic acid is excreted via urine.
Half life
Approximately 1 to 1.5 hours for both cefuroxime and clavulanic acid.
Absorption
Cefuroxime Axetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is enhanced with food. Clavulanic acid is also well absorbed orally.
Metabolism
Cefuroxime is not metabolized. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, clavulanic acid, any other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity reaction to any other beta-lactam antibiotic (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Increases plasma concentrations and prolongs the half-life of cefuroxime by reducing renal tubular secretion.
Oral Contraceptives
May decrease the efficacy of oral contraceptives. Advise use of alternative non-hormonal contraception.
Oral anticoagulants
May increase INR in patients receiving warfarin. Careful monitoring is advised.
Antacids and H2-receptor antagonists
Can decrease the bioavailability of cefuroxime. Administer at least 1-2 hours before or after.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms include gastrointestinal disturbances (nausea, vomiting, diarrhea) and possibly neurotoxic effects such as seizures. Treatment is symptomatic and supportive. Hemodialysis and peritoneal dialysis can remove cefuroxime and clavulanic acid from the body.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Cefuroxime and clavulanic acid are excreted in breast milk; use with caution in breastfeeding mothers and monitor infants for potential adverse effects like diarrhea or candidiasis.

YesClinical Trials
Extensive clinical trials have established the efficacy and safety of cefuroxime and clavulanic acid in treating various bacterial infections. Ongoing post-marketing surveillance continues to monitor for rare adverse events and efficacy.
Lab Monitoring
- Monitor renal function (creatinine, BUN) in patients with pre-existing renal impairment or during prolonged high-dose therapy.
- Liver function tests (ALT, AST, alkaline phosphatase) may be monitored in patients with hepatic dysfunction or on prolonged therapy.
- Complete blood count (CBC) for prolonged therapy.
Doctor Notes
- Emphasize the importance of completing the full antibiotic course to prevent resistance.
- Advise patients on potential gastrointestinal side effects and the benefit of taking with food.
- Counsel on signs and symptoms of C. difficile-associated diarrhea.
Patient Guidelines
- Complete the full course of medication, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Take with food to improve absorption and reduce stomach upset.
- Do not share this medication with others, and do not use it for viral infections.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
