Overview of the medicine
Cefuroxime axetil is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typical dose is 250 mg to 500 mg twice daily, depending on the severity and type of infection. For most infections, 250 mg twice daily. For severe infections or lower respiratory tract infections, 500 mg twice daily. Duration usually 5-10 days.
Elderly
Similar to adult dosage, but caution should be exercised in patients with renal impairment. Renal function should be monitored.
Renal_impairment
Dosage adjustment is required for patients with severe renal impairment (creatinine clearance < 30 mL/min). For CrCl 10-29 mL/min, 250 mg once daily. For CrCl < 10 mL/min, 250 mg every 48 hours. Hemodialysis patients should take an additional dose after each dialysis.
How to Take
Take orally, preferably with food to enhance absorption and reduce gastrointestinal discomfort. Do not crush or chew the tablet.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached approximately 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged in urine by glomerular filtration and tubular secretion, within 24 hours.
Half life
Approximately 1.2 to 1.5 hours.
Absorption
Well absorbed from the gastrointestinal tract after oral administration, enhanced with food. Bioavailability is approximately 37% to 52%.
Metabolism
Hydrolyzed by esterases in the intestinal mucosa and blood to release cefuroxime. Cefuroxime itself is not significantly metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems)
Drug Interactions
Warfarin
Although not definitive, some cephalosporins may potentiate the anticoagulant effect of warfarin. INR monitoring is advisable.
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral Contraceptives
Cefuroxime may affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives.
Antacids/H2 blockers
Drugs that reduce gastric acidity may reduce the bioavailability of cefuroxime axetil. It is recommended to take cefuroxime axetil at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdosage of cephalosporins can cause cerebral irritation leading to convulsions. Management includes symptomatic and supportive treatment. Hemodialysis and peritoneal dialysis may be useful in enhancing elimination.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not revealed evidence of impaired fertility or harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefuroxime is excreted in human milk in small amounts; use with caution in nursing mothers.
