Description
Overview of the medicine
Fusicort cream is a combination of a potent corticosteroid (Betamethasone) and an antibiotic (Fusidic Acid). It is used to treat inflamed skin conditions where bacterial infection is present or likely.
Uses & Indications
Dosage
Adults
Apply thinly to the affected area 2-3 times daily. Treatment should not exceed 1-2 weeks.
Elderly
No specific dosage adjustments required; use with caution for prolonged periods.
Renal_impairment
No dosage adjustment required for topical application due to minimal systemic absorption.
How to Take
For topical application only. Apply a thin layer to the affected skin area after washing hands. Gently rub in. Avoid contact with eyes.
Mechanism of Action
Fusidic acid inhibits bacterial protein synthesis by interfering with the translocation of elongation factor G, primarily against Gram-positive bacteria, especially Staphylococcus aureus. Betamethasone is a potent corticosteroid that provides anti-inflammatory, antipruritic, and vasoconstrictive effects by inhibiting the release of inflammatory mediators.
Pharmacokinetics
Onset
Anti-inflammatory and antimicrobial effects usually begin within hours to days of application.
Excretion
Systemically absorbed fusidic acid is excreted mainly via bile. Betamethasone and its metabolites are primarily excreted via the kidneys.
Half life
Systemic half-life is not clinically significant for topical application due to minimal absorption.
Absorption
Limited systemic absorption for both components when applied topically to intact skin. Absorption may increase with occlusive dressings or on damaged skin.
Metabolism
Any systemically absorbed fusidic acid is primarily metabolized in the liver and excreted in the bile. Betamethasone is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to fusidic acid, betamethasone, or any excipients
- Primary skin infections caused by fungi, viruses (e.g., herpes simplex, chickenpox), or mycobacteria (e.g., tuberculosis of the skin)
- Perioral dermatitis and rosacea
- Acne vulgaris
- Ulcerative conditions
Drug Interactions
Systemic drug interactions
Systemic interactions are unlikely due to minimal systemic absorption from topical application. However, caution is advised if other corticosteroids are used concurrently.
Storage
Store below 25°C to 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute systemic problems. Prolonged or excessive use over large areas may lead to systemic corticosteroid effects, such as Cushing's syndrome, or adrenal suppression. Treatment is symptomatic.
Pregnancy & Lactation
Use during pregnancy and lactation should only be considered if the potential benefit outweighs the potential risk to the fetus or infant. Avoid extensive or prolonged use, especially on large surface areas or under occlusive dressings.

Lab Monitoring
- Not routinely required for topical use. Adrenal function tests may be considered in cases of extensive or prolonged use, especially in children.
Doctor Notes
- Emphasize short-term use (max 2 weeks) to minimize corticosteroid side effects and antibiotic resistance.
- Advise patients on appropriate application technique to ensure efficacy and avoid overuse.
- Differentiate from purely fungal or viral infections where this combination is contraindicated.
