Description
Overview of the medicine
G-Benzathine Penicillin is a long-acting form of penicillin used to treat various bacterial infections, primarily syphilis, and for the prophylaxis of rheumatic fever and streptococcal infections. It is administered via deep intramuscular injection.
Uses & Indications
Dosage
Adults
Syphilis: 2.4 million units IM as a single dose. Rheumatic fever prophylaxis: 1.2 million units IM every 3-4 weeks. Streptococcal pharyngitis: 1.2 million units IM as a single dose.
Elderly
No specific dose adjustment for elderly patients based on age alone, but monitor renal function.
Renal_impairment
Dosage adjustment may be necessary in patients with severe renal impairment due to reduced excretion.
How to Take
Administer by deep intramuscular injection, preferably in the upper outer quadrant of the buttock in adults or the midlateral thigh in infants and young children. DO NOT administer intravenously, subcutaneously, or intra-arterially.
Mechanism of Action
It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It is bactericidal against susceptible organisms.
Pharmacokinetics
Onset
Delayed onset of action due to slow absorption (peak serum levels typically occur 12-24 hours post-injection).
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion.
Half life
Variable, typically 3-7 days, depending on dose and individual factors, allowing for prolonged therapeutic effect.
Absorption
Slowly absorbed from the intramuscular injection site, providing sustained low serum concentrations over several days or weeks.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to penicillin or other beta-lactam antibiotics (e.g., cephalosporins)
- •Do not administer intravenously (can cause cardiorespiratory arrest and death)
Drug Interactions
Methotrexate
Reduced renal clearance of methotrexate, potentially increasing toxicity.
Tetracyclines
May antagonize the bactericidal effect of penicillin; avoid concomitant use.
Oral anticoagulants (e.g., Warfarin)
May potentiate the effects of anticoagulants; monitor INR.
Storage
Store unconstituted product at room temperature (20-25°C), protected from light and moisture. Do not freeze.
Overdose
Overdose may lead to neuromuscular excitability or convulsions, especially in patients with severe renal impairment. Treatment is supportive; hemodialysis may be beneficial in severe cases.
Pregnancy & Lactation
Pregnancy category B: Generally considered safe during pregnancy. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for adverse effects like diarrhea or rash.
