Description
Overview of the medicine
Cefradine is a first-generation cephalosporin antibiotic used to treat a wide range of bacterial infections, particularly in pediatric patients, in the form of a 125 mg/5 ml oral suspension.
Uses & Indications
Dosage
Adults
Not for adult use in this formulation (Pediatric Drop). Adults typically use higher strength capsules/tablets.
Elderly
Not for elderly use in this formulation. Dosage in elderly with normal renal function is similar to adults; adjust for renal impairment.
Pediatric
Children 25-50 mg/kg/day in 2-4 divided doses. For severe infections, up to 100 mg/kg/day in divided doses. Max 4 g/day. (Specific for 125mg/5ml: e.g., 0.5 ml - 2 ml per dose depending on weight/age and infection severity).
Renal_impairment
Dosage adjustment is necessary. Consult prescribing information for specific creatinine clearance guidelines.
How to Take
This medication is for oral use. Shake the bottle well before each use. Administer the prescribed dose using the provided dropper or measuring spoon. It can be taken with or without food. To minimize stomach upset, it may be taken with food or milk.
Mechanism of Action
Cefradine acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly by the kidneys through glomerular filtration and tubular secretion. Approximately 60-90% of a dose is excreted unchanged in the urine within 6 hours.
Half life
Approximately 0.5 to 2 hours, slightly prolonged in patients with impaired renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are usually reached within 1 hour.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefradine, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity).
Drug Interactions
Metformin
Cephalosporins may reduce renal clearance of metformin, increasing metformin levels.
Probenecid
May decrease renal tubular secretion of cefradine, resulting in increased and prolonged plasma concentrations of cefradine.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity, especially in patients with pre-existing renal impairment.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring monitoring of INR and dose adjustment.
Storage
Store powder for suspension at room temperature (below 25°C) in a dry place, away from light and moisture. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days (check specific product leaflet). Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, epigastric distress, and convulsions. Management is primarily supportive. Hemodialysis may be useful in removing cefradine from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefradine is excreted in breast milk in small amounts; use with caution in nursing mothers.

As a well-established generic antibiotic, extensive clinical trials have been conducted historically to demonstrate its efficacy and safety. Current usage is based on this historical data and post-market surveillance.
Lab Monitoring
- Periodic assessment of renal function is recommended during prolonged therapy.
- Complete blood counts should be monitored during prolonged therapy.
- Liver function tests may be monitored if clinically indicated.
Doctor Notes
- Emphasize adherence to the full course of therapy to prevent resistance.
- Consider renal function in pediatric patients, especially neonates and infants, for appropriate dosing.
- Monitor for signs of allergic reactions, particularly in patients with a history of penicillin allergy.
Patient Guidelines
- Complete the full course of medication, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Do not save medication for future use or share it with others.
- Shake the bottle well before each use.
- Use the provided measuring device for accurate dosing.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. However, if it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefradine generally does not impair the ability to drive or operate machinery. However, if dizziness occurs, patients should avoid such activities.
Lifestyle Advice
- Ensure adequate hydration by drinking plenty of fluids.
- Maintain good personal hygiene to prevent the spread of infection.
- Follow a balanced diet.
