Overview of the medicine
Losartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to protect the kidneys from damage due to diabetes.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mg once daily, adjusted based on blood pressure response, maximum 100 mg daily.
Elderly
No initial dosage adjustment typically required, but consider lower starting dose (25 mg) in patients over 75 years.
Renal_impairment
No initial dosage adjustment required for mild to moderate renal impairment. For severe renal impairment, consider lower starting dose (25 mg).
How to Take
Orally, with or without food, preferably at the same time each day.
Mechanism of Action
Blocks the binding of angiotensin II to the AT1 receptor, preventing vasoconstriction and aldosterone secretion, thus lowering blood pressure.

Onset
About 6 hours (peak effect)
Excretion
About 35% in urine and 60% in feces (as unchanged drug and metabolites).
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed from the gastrointestinal tract, undergoes first-pass metabolism.
Metabolism
Extensively metabolized by CYP2C9 and CYP3A4 to an active carboxylic acid metabolite (E-3174).
Side Effects
Contraindications
- •Pregnancy (especially second and third trimesters)
- •Hypersensitivity to losartan or any component of the formulation
- •Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 mL/min/1.73m²)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
NSAIDs (e.g., ibuprofen, naproxen)
May attenuate the antihypertensive effect and increase the risk of renal impairment.
Diuretics and other antihypertensives
Additive hypotensive effects.
Potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypotension, tachycardia, and possibly bradycardia. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category D (second and third trimesters). Avoid during lactation as it may pass into breast milk.
