Description
Overview of the medicine
g-Metoprolol 50 mg Tablet is a beta-blocker used to treat high blood pressure, angina (chest pain), and to improve survival after a heart attack. It works by slowing down the heart rate and relaxing blood vessels.
Uses & Indications
Dosage
Adults
Hypertension: 100-400 mg daily in single or divided doses. Angina: 100-400 mg daily in 2 divided doses. Post-MI: 50 mg every 6 hours for 48 hours, then 100 mg twice daily.
Elderly
Start with lower doses and titrate carefully, often similar to adult doses but with caution.
Renal_impairment
No dose adjustment usually required for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally, with or immediately after food. Do not chew or crush extended-release tablets.
Mechanism of Action
Metoprolol is a cardioselective beta-1 adrenergic receptor blocker. It selectively blocks beta-1 receptors in the heart, reducing heart rate, cardiac output, and blood pressure.
Pharmacokinetics
Onset
Oral: 15 minutes; IV: Immediate.
Excretion
Primarily excreted in urine (approximately 95% as metabolites, 5% as unchanged drug).
Half life
3-7 hours.
Absorption
Rapidly and completely absorbed from the GI tract (oral bioavailability 40-50% due to first-pass metabolism).
Metabolism
Extensively metabolized in the liver by CYP2D6.
Side Effects
Contraindications
- Sinus bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Decompensated heart failure
- Sick sinus syndrome (without a pacemaker)
Drug Interactions
NSAIDs
May reduce antihypertensive effect.
Digoxin
May increase bradycardia.
Calcium channel blockers (e.g., verapamil, diltiazem)
May increase risk of bradycardia and hypotension.
Catecholamine-depleting drugs (e.g., reserpine, guanethidine)
May have an additive effect causing hypotension and bradycardia.
Storage
Store at room temperature (15-30°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms include severe bradycardia, hypotension, cardiogenic shock, bronchospasm, heart failure. Management involves symptomatic and supportive care, e.g., atropine for bradycardia, glucagon for cardiac depression, vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Metoprolol is excreted in breast milk; caution advised.
