Description
Overview of the medicine
Phenoxymethylpenicillin, commonly known as Penicillin V, is an antibiotic used to treat a variety of bacterial infections, including those of the throat, skin, and dental infections. It is also used for the prevention of rheumatic fever.
Uses & Indications
Dosage
Adults
250-500 mg every 6-8 hours, depending on the severity of the infection. For prophylaxis of rheumatic fever, 250 mg twice daily.
Elderly
Same as adults, but caution is advised in patients with impaired renal function.
Renal_impairment
Dose reduction may be necessary. Consult a physician.
How to Take
Take the tablet orally with a glass of water, with or without food. It is crucial to complete the full prescribed course of treatment, even if symptoms improve, to prevent the development of antibiotic resistance.
Mechanism of Action
Penicillin V works by interfering with the synthesis of the bacterial cell wall, leading to the lysis and death of the bacteria. It is bactericidal against susceptible organisms.
Pharmacokinetics
Onset
Rapid.
Excretion
Excreted primarily by the kidneys (60-80% as unchanged drug) within 4-5 hours.
Half life
Approximately 30 to 60 minutes.
Absorption
Well absorbed orally; acid-stable. Peak plasma concentrations are reached within 30-60 minutes.
Metabolism
Partially metabolized in the liver (10-30%).
Side Effects
Contraindications
- Known hypersensitivity to penicillins, cephalosporins, or any component of the formulation.
- Severe asthma or history of severe allergic reactions.
Drug Interactions
Warfarin
May increase the anticoagulant effect of warfarin, requiring close monitoring of INR.
Methotrexate
May increase methotrexate levels and toxicity due to reduced renal clearance.
Tetracyclines
May decrease the bactericidal effect of penicillin V.
Oral contraceptives
Evidence is weak, but some penicillins may reduce the efficacy of oral contraceptives.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and rarely, neurological effects such as convulsions, especially in patients with impaired renal function. Treatment is supportive and symptomatic; hemodialysis may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Generally considered safe during pregnancy. Compatible with breastfeeding, as only small amounts are excreted in breast milk.
