Silver Sulfadiazine 1% cream is an antimicrobial agent indicated for the prevention and treatment of wound sepsis in patients with second- and third-degree burns.
Uses & Indications
Dosage
Adults
Apply topically to the affected area once or twice daily, typically a layer 2-4 mm thick. Re-apply as needed if cream is removed by activity or water.
Elderly
Same as adults, but monitor for systemic absorption if extensive application is used.
Renal_impairment
Use with caution, especially if extensive absorption is possible due to large application areas, as sulfadiazine is renally excreted.
How to Take
Cleanse and debride the burn wound thoroughly prior to application. Apply a thin layer (2-4 mm thick) directly to the affected area using a sterile gloved hand or spatula. The cream should be re-applied daily and whenever necessary to maintain continuous coverage.
Mechanism of Action
Silver sulfadiazine dissociates into silver ions and sulfadiazine. Silver ions exert antimicrobial effects by binding to bacterial DNA and cell walls, disrupting their integrity. Sulfadiazine inhibits bacterial folic acid synthesis by interfering with dihydropteroate synthase, leading to bacteriostasis.

Systemically absorbed sulfadiazine and silver are primarily excreted via the kidneys.

Systemically absorbed sulfadiazine has a half-life of 10-12 hours.
Absorption
Minimal systemic absorption (up to 10% of sulfadiazine and 1% of silver may be absorbed from large burn areas). Primarily localized action.
Metabolism
Systemically absorbed sulfadiazine is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to silver sulfadiazine, sulfonamides, or any component of the cream.
- •Premature infants or neonates (risk of kernicterus).
- •Pregnancy near term (risk of kernicterus in the infant).
- •Breastfeeding women of infants less than 2 months old.
- •Patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency (risk of hemolysis).
Drug Interactions
Cimetidine
May increase the incidence of leukopenia when administered concurrently with silver sulfadiazine.
Proteolytic enzymes
Silver may inactivate proteolytic enzymes (e.g., collagenase, papain, sutilains) used for enzymatic debridement.
Systemic sulfonamides
Potential for additive effects if significant systemic absorption of sulfadiazine occurs.
Storage
Store at room temperature (below 25-30°C). Do not freeze. Keep away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Systemic toxicity from topical overdose is rare due to minimal absorption. If accidental ingestion or excessive application over large areas leads to systemic effects, manage symptomatically and supportively (e.g., fluid administration, monitoring of renal function, blood counts).
Pregnancy & Lactation
Pregnancy Category B, but contraindicated near term due to potential for kernicterus in the infant. Use only if potential benefit outweighs risk. Contraindicated in breastfeeding women of infants less than 2 months old.
