Description
Overview of the medicine
Pyridoxine (Vitamin B6) is a water-soluble vitamin involved in various metabolic processes, including amino acid metabolism, neurotransmitter synthesis, and red blood cell formation. It is essential for proper nerve function and immune health.
Uses & Indications
Dosage
Adults
Deficiency: 10-20 mg daily for 3 weeks, then 2-5 mg daily. Isoniazid prophylaxis: 10 mg daily. Pregnancy nausea: 10-25 mg, 3-4 times daily.
Elderly
Same as adult dosage, but monitor for potential decreased absorption.
Renal_impairment
No specific dose adjustment generally needed for oral supplementation at typical doses.
How to Take
Oral administration, with or without food. Can be crushed if needed.
Mechanism of Action
Pyridoxine acts as a coenzyme in numerous metabolic reactions, particularly in amino acid and protein metabolism. It is converted to its active form, pyridoxal 5'-phosphate (PLP), which is crucial for over 100 enzyme reactions, including transamination, decarboxylation, and racemization. PLP is also involved in the synthesis of neurotransmitters like serotonin, dopamine, and GABA.
Pharmacokinetics
Onset
Varies, symptomatic relief can be seen within days to weeks for deficiency.
Excretion
Excreted renally as 4-pyridoxic acid and other inactive metabolites.
Half life
Approximately 15-20 days (for PLP).
Absorption
Readily absorbed from the gastrointestinal tract, primarily in the jejunum.
Metabolism
Converted to its active form, pyridoxal 5'-phosphate (PLP), primarily in the liver.
Side Effects
Contraindications
- Hypersensitivity to pyridoxine or any component of the formulation.
Drug Interactions
Levodopa
Pyridoxine can reduce the effectiveness of levodopa (without carbidopa).
Phenobarbital, Phenytoin
Pyridoxine may decrease serum concentrations of these anticonvulsants.
Isoniazid, Penicillamine, Hydralazine, Cycloserine, Oral contraceptives
These drugs can increase pyridoxine requirements or lead to deficiency.
Storage
Store below 30°C in a cool, dry place, away from light and moisture.
Overdose
Overdose is rare due to water solubility. Very high chronic doses (e.g., >200 mg/day for months) can cause severe peripheral neuropathy, ataxia, and skin lesions. Treatment is symptomatic and supportive, discontinuation of the supplement.
Pregnancy & Lactation
Generally considered safe at recommended daily allowance (RDA) doses. Higher doses may be used for nausea and vomiting in pregnancy under medical supervision. Pyridoxine is excreted in breast milk; generally safe at recommended doses.

WHO Essential Medicine
YesClinical Trials
Extensive clinical use has established its efficacy and safety. Specific trials often focus on specific indications like pregnancy nausea or drug-induced neuropathy.
Lab Monitoring
- Serum pyridoxine levels (for deficiency or toxicity investigation).
Doctor Notes
- Essential for patients on Isoniazid to prevent neuropathy.
- Consider deficiency in alcoholic patients, malnourished individuals, and those on certain medications.
Patient Guidelines
- Take as directed by your doctor or pharmacist.
- Do not exceed recommended doses, especially for long periods.
- Report any unusual side effects, especially numbness or tingling.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double dose.
Driving Precautions
Generally safe to drive as it does not typically cause drowsiness or impaired judgment at therapeutic doses. High doses causing neuropathy might impair driving ability.
Lifestyle Advice
- Maintain a balanced diet rich in whole grains, fruits, vegetables, and lean protein.
- Avoid excessive alcohol consumption, which can interfere with B6 metabolism.
