Description
Overview of the medicine
Gabapentin Extended-Release 825 mg tablets are used to treat certain types of nerve pain (neuropathic pain), such as postherpetic neuralgia, which is nerve pain that occurs after shingles. It may also be used in some cases for restless legs syndrome.
Uses & Indications
Dosage
Adults
For Postherpetic Neuralgia: Initial dose of 300 mg once daily, titrated up to 1800 mg once daily. For Restless Legs Syndrome: Initial dose of 300 mg once daily, titrated up to 1800 mg once daily. Always follow physician's instructions.
Elderly
Dosage may need to be adjusted based on renal function. Close monitoring for side effects is recommended.
Renal_impairment
Dosage must be adjusted in patients with impaired renal function (creatinine clearance < 60 mL/min). Specific dose reduction guidelines should be followed based on creatinine clearance.
How to Take
Take orally, once daily, preferably with evening meal. Swallow the tablet whole; do not split, crush, or chew. Do not take with antacids containing aluminum or magnesium, allow at least 2 hours between doses.
Mechanism of Action
The exact mechanism of action of gabapentin is not fully understood. It is structurally related to the neurotransmitter gamma-aminobutyric acid (GABA) but does not bind to GABAA or GABAB receptors, nor does it inhibit GABA uptake or degradation. It is thought to exert its effects by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby modulating the release of excitatory neurotransmitters.
Pharmacokinetics
Onset
Therapeutic effects may be observed within a few days to a few weeks, depending on the indication and individual response.
Excretion
Excreted primarily unchanged by the kidneys. Renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 5 to 7 hours in individuals with normal renal function.
Absorption
Bioavailability is not dose-proportional; it decreases as the dose increases. Peak plasma concentrations are reached 6-8 hours after oral administration of the extended-release formulation. Food has minimal effect on absorption.
Metabolism
Gabapentin is not significantly metabolized in humans. It does not induce hepatic microsomal enzymes.
Side Effects
Contraindications
- Hypersensitivity to gabapentin or any component of the formulation
- Acute pancreatitis (rarely reported)
Drug Interactions
Opioids
May increase CNS depression and respiratory depression. Use with caution; consider dose reduction of opioid.
Antacids (containing aluminum and magnesium)
Can reduce gabapentin bioavailability. Separate administration by at least 2 hours.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, double vision, slurred speech, drowsiness, lethargy, and mild diarrhea. Management is primarily supportive; hemodialysis can be used to remove gabapentin in patients with severe renal impairment or life-threatening symptoms.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; use with caution in nursing mothers, considering the benefits of breastfeeding and the potential risks.

WHO Essential Medicine
YesClinical Trials
Ongoing clinical trials are exploring gabapentin's efficacy for other types of neuropathic pain and chronic pain conditions. Specific trials for 825 mg ER formulation usually focus on optimizing dosing regimens and long-term safety.
Lab Monitoring
- Monitor renal function (creatinine clearance) in patients with kidney impairment.
- Monitor for signs of depression, suicidal thoughts, or unusual behavioral changes.
Doctor Notes
- Gabapentin ER 825 mg is indicated for PHN and RLS; not interchangeable with IR formulations for epilepsy due to different pharmacokinetic profiles.
- Renal dose adjustment is crucial for patients with CrCl < 60 mL/min.
- Educate patients on potential CNS depressant effects (dizziness, somnolence) and the importance of not stopping treatment abruptly.
Patient Guidelines
- Do not stop taking Gabapentin abruptly; it should be tapered gradually to avoid withdrawal symptoms.
- Report any new or worsening depression, suicidal thoughts, or unusual changes in mood or behavior to your doctor immediately.
- This medicine may cause dizziness or drowsiness; avoid driving or operating machinery until you know how it affects you.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is close to the time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Gabapentin may cause dizziness, drowsiness, and impaired coordination. Patients should be advised not to drive or operate complex machinery until they have gained sufficient experience with Gabapentin to gauge whether it adversely affects their ability to drive or operate complex machinery.
Lifestyle Advice
- Avoid alcohol consumption while taking gabapentin as it can increase CNS depressant effects.
- Maintain good hydration.
