Description
Overview of the medicine
Gabantin 300 mg Tablet contains Gabapentin, an anticonvulsant used to treat various types of seizures and neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
For neuropathic pain, typically initiated at 300 mg on day 1, 300 mg twice daily on day 2, and 300 mg three times daily on day 3, then titrated up to a maximum of 1800-3600 mg/day in divided doses. For epilepsy, usually 300 mg three times daily, adjusted according to response and tolerability, up to a maximum of 3600 mg/day.
Elderly
Lower initial doses and slower titration are recommended due to potential age-related decrease in renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl 30-59 mL/min: 200-700 mg twice daily. For CrCl 15-29 mL/min: 200-700 mg once daily. For CrCl <15 mL/min: 100-300 mg once daily. Hemodialysis patients require supplemental doses after dialysis.
How to Take
Gabantin tablets should be swallowed whole with water, with or without food. Do not chew, crush, or break the tablet.
Mechanism of Action
Gabapentin's precise mechanism is unknown, but it is thought to bind to the alpha-2-delta subunit of voltage-gated calcium channels, leading to reduced neuronal excitability and neurotransmitter release.
Pharmacokinetics
Onset
Therapeutic effects may be observed within a few days to weeks of starting treatment or dose titration.
Excretion
Excreted unchanged by the kidneys. Dose adjustment is necessary in patients with impaired renal function.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Oral bioavailability is not dose-proportional; it decreases with increasing dose due to saturable absorption via an L-amino acid transporter.
Metabolism
Not appreciably metabolized in humans; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Gabapentin or any component of the formulation.
Drug Interactions
Alcohol
Increases CNS depression. Avoid concomitant use.
Opioids (e.g., Morphine)
May increase CNS depressant effects of Gabapentin, including somnolence and respiratory depression. Use with caution, especially in patients with respiratory risk factors.
Antacids containing aluminum and magnesium
May decrease Gabapentin bioavailability; take Gabapentin at least 2 hours after antacids.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, double vision, slurred speech, drowsiness, lethargy, and mild diarrhea. Management is supportive, including gastric lavage or activated charcoal if recent ingestion, and symptomatic treatment. Hemodialysis can remove Gabapentin.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in human milk; caution should be exercised when administered to a nursing woman.

WHO Essential Medicine
YesClinical Trials
Gabapentin has undergone extensive clinical trials demonstrating its efficacy in treating partial seizures and postherpetic neuralgia. Ongoing research continues to explore its potential uses and long-term effects.
Lab Monitoring
- No specific routine lab monitoring is required for Gabapentin in patients with normal renal function. Renal function (creatinine clearance) should be monitored in patients with kidney impairment and elderly patients.
Doctor Notes
- Emphasize gradual titration to minimize adverse effects and ensure patient tolerability.
- Monitor patients for signs of respiratory depression, especially if co-administered with opioids or other CNS depressants.
- Advise patients on the importance of not discontinuing the medication abruptly due to seizure risk.
Patient Guidelines
- Do not stop taking Gabantin abruptly, as this may increase seizure frequency.
- Report any changes in mood, behavior, or suicidal thoughts to your doctor immediately.
- Avoid driving or operating heavy machinery until you know how Gabantin affects you, as it may cause dizziness or drowsiness.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Gabantin may cause dizziness, drowsiness, and blurred vision, which can impair your ability to drive or operate machinery. Exercise caution and avoid such activities until you are certain how the medication affects you.
Lifestyle Advice
- Avoid alcohol consumption while taking Gabantin, as it can intensify side effects like drowsiness and dizziness.
- Maintain regular follow-up appointments with your doctor to monitor your condition and adjust dosage if necessary.
