Overview of the medicine
Gabapre (Gabapentin) is an anticonvulsant and neuropathic pain agent used to treat partial seizures, postherpetic neuralgia, and restless legs syndrome.
Uses & Indications
Dosage
Adults
For partial seizures: Initial dose 300 mg three times daily (TID), titrated up to 900-3600 mg/day. For postherpetic neuralgia: Initial 300 mg on Day 1, 300 mg BID on Day 2, 300 mg TID on Day 3, titrated up to 1800 mg/day. For restless legs syndrome: Initial 300 mg once daily, titrated up to 1800 mg/day.
Elderly
Lower initial doses and careful titration are recommended due to potential age-related decrease in renal function.
Renal_impairment
Dose adjustment is necessary based on creatinine clearance. Lower doses are required for patients with impaired renal function.
How to Take
Take orally with or without food. Capsules should be swallowed whole. Extended-release tablets should not be crushed, chewed, or cut.
Mechanism of Action
Its exact mechanism of action is unknown, but it is thought to modulate the release of excitatory neurotransmitters by binding to the alpha-2-delta subunit of voltage-gated calcium channels. It does not bind to GABA receptors.

Side Effects
Contraindications

Drug Interactions
Antacids
May decrease gabapentin bioavailability; take gabapentin at least 2 hours after antacid administration.
Opioids (e.g., Morphine)
May increase gabapentin levels and enhance CNS depressive effects. Monitor for signs of sedation and respiratory depression.
Alcohol and other CNS depressants
May increase CNS depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, ataxia, lethargy, and mild diarrhea. Management is primarily supportive; hemodialysis can be used to remove gabapentin.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin has been shown to be teratogenic in animals. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; use with caution in breastfeeding mothers.
