Description
Overview of the medicine
Gabarol CR 165 mg Tablet is a controlled-release formulation of Gabapentin, an anticonvulsant and neuropathic pain agent. It is used to treat nerve pain (neuropathic pain) and certain types of seizures.
Uses & Indications
Dosage
Adults
For postherpetic neuralgia: Typically initiated at 300 mg once daily, titrated up to 1800-3600 mg/day as a single daily dose (Gabarol CR 165mg would be part of a titration schedule or specific lower doses). For partial seizures: Doses can range from 900 mg to 3600 mg/day in divided doses, or once daily for CR formulations.
Elderly
Dose adjustments may be necessary due to potential age-related decline in renal function.
Renal_impairment
Dosage must be adjusted in patients with impaired renal function (creatinine clearance < 60 mL/min). Lower doses and/or extended intervals are required.
How to Take
Take Gabarol CR tablets orally, preferably with evening meal. Swallow whole; do not split, crush, or chew. Do not take with antacids; separate dosing by at least 2 hours.
Mechanism of Action
Gabapentin's precise mechanism is unknown, but it is thought to involve modulating the alpha-2-delta subunit of voltage-gated calcium channels, leading to reduced neurotransmitter release. It does not bind to GABA-A, GABA-B, or benzodiazepine receptors.
Pharmacokinetics
Onset
Varies by indication and individual; typically within hours to days for noticeable effects.
Excretion
Excreted unchanged by the kidneys. Renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Not dose-proportional; absorption is saturable. Bioavailability decreases with increasing dose. The CR formulation aims to improve consistency and duration of absorption.
Metabolism
Not significantly metabolized in humans. Does not induce or inhibit hepatic drug-metabolizing enzymes.
Side Effects
Contraindications
- Hypersensitivity to gabapentin or any component of the formulation.
- Acute pancreatitis.
Drug Interactions
Opioids
May increase CNS depression and respiratory depression.
Antacids
Reduces gabapentin bioavailability. Separate administration by at least 2 hours.
Morphine
Increases gabapentin concentrations. Monitor for CNS depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, double vision, slurred speech, drowsiness, lethargy, and mild diarrhea. Management is supportive, including gastric lavage if recent ingestion, and hemodialysis may be considered in severe cases, especially with renal impairment.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Gabapentin is excreted in human milk; use with caution in nursing mothers.

Lab Monitoring
- Renal function (creatinine clearance) in patients with impaired kidneys.
- Monitor for signs of CNS depression or changes in mood/behavior.
Doctor Notes
- Counsel patients on potential CNS depressant effects and the importance of not abruptly discontinuing therapy.
- Monitor renal function, especially in elderly patients or those with pre-existing kidney impairment.
- Be aware of the potential for misuse and abuse, particularly in populations at risk.
