Description
Overview of the medicine
Gemicin is a macrolide antibiotic used to treat a wide range of bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections: 320 mg once daily for 3-5 days. For uncomplicated Chlamydia: A single dose of 960 mg (3 tablets of 320 mg). Dosage may vary based on infection severity and type.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with pre-existing cardiac conditions due to potential for QT prolongation.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution and monitor in severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Gemicin tablets should be taken orally, with or without food. It is recommended to take the medicine at approximately the same time each day to maintain consistent drug levels.
Mechanism of Action
Gemicin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thus preventing the translation of messenger RNA (mRNA) and bacterial growth.
Pharmacokinetics
Onset
Clinical effects typically observed within 2-3 hours of administration.
Excretion
Mainly excreted unchanged in bile, with only a small portion (approximately 6%) excreted in urine over 72 hours.
Half life
The terminal elimination half-life is prolonged, ranging from 2 to 4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver by N-demethylation into inactive metabolites. No significant first-pass metabolism.
Side Effects
Contraindications
- •Hypersensitivity to Gemicin, azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice or hepatic dysfunction associated with prior use of Gemicin or other macrolide antibiotics.
Drug Interactions
Digoxin
May increase serum digoxin levels. Monitor digoxin levels and adjust dosage if necessary.
Warfarin
Potentiates the anticoagulant effect. Close monitoring of Prothrombin Time/INR is recommended.
Nelfinavir
Increased Gemicin serum concentrations. Co-administration should be monitored for increased side effects.
Antacids (containing aluminum or magnesium)
Can decrease the absorption of Gemicin. Administer Gemicin at least 2 hours before or 4 hours after antacids.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Gemicin overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive. Gastric lavage and general supportive measures may be necessary.
Pregnancy & Lactation
Pregnancy Category B. Use Gemicin during pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. It is excreted in human milk; caution should be exercised when administered to a nursing woman.
