Genac-Plus 75 mg Injection contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID). It is used to relieve various types of pain, inflammation, and fever.
Uses & Indications
Dosage
Adults
Typically 75 mg once daily by deep intramuscular injection. In severe cases, 75 mg twice daily may be given for a short duration. Max 150 mg/day for 2 days. Then switch to oral.
Elderly
Use with caution and at the lowest effective dose. Increased risk of side effects.
Renal_impairment
Use with caution. Dosage reduction may be necessary in severe impairment. Not recommended in severe renal failure.
How to Take
For deep intramuscular injection, typically into the gluteal muscle. For intravenous infusion, it must be diluted and administered slowly under hospital supervision.
Mechanism of Action
Diclofenac Sodium inhibits cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins play a key role in mediating pain, inflammation, and fever.

Onset
Onset of pain relief typically occurs within 15-30 minutes.
Excretion
Approximately 60% of the dose is excreted via the kidneys and 35% via bile/feces as metabolites.
Half life
Plasma elimination half-life is 1-2 hours; half-life in synovial fluid is 3-8 hours.
Absorption
Rapid and complete absorption after intramuscular administration. Peak plasma concentrations are reached within 10-30 minutes.
Metabolism
Primarily metabolized in the liver (mainly by CYP2C9, 2C19, and 3A4) to inactive hydroxylated and glucuronidated metabolites.
Side Effects
Contraindications
- •Hypersensitivity to diclofenac or any NSAID
- •Active gastrointestinal bleeding or peptic ulcer
- •History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
- •Severe heart failure
- •Severe renal or hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Methotrexate
Increased methotrexate toxicity.
Lithium/Digoxin
Increased plasma levels of lithium/digoxin.
Cyclosporine/Tacrolimus
Increased risk of nephrotoxicity.
Anticoagulants/Antiplatelets
Increased risk of bleeding.
Diuretics/ACE Inhibitors/ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus and renal dysfunction in the fetus. Use with caution in the first and second trimesters only if clearly needed. Not recommended during lactation as diclofenac passes into breast milk.
