Ganciclovir is a synthetic guanine derivative that is phosphorylated to ganciclovir triphosphate by viral and cellular kinases. Ganciclovir triphosphate inhibits viral DNA synthesis by competitive inhibition of viral DNA polymerase and by direct incorporation into viral DNA, leading to chain termination.

Ocular overdose is unlikely to cause systemic toxicity due to minimal absorption. If excessive amounts are applied, flush the eye with lukewarm water.

Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if ganciclovir is excreted in human milk after ophthalmic administration; caution should be exercised when administered to a nursing mother.
