Description
Overview of the medicine
Domperidone is a dopamine antagonist with prokinetic and antiemetic properties. It is used to relieve nausea, vomiting, and gastrointestinal motility disorders.
Uses & Indications
Dosage
Adults
Adults and adolescents (≥12 years and ≥35 kg): 10 ml (10 mg) up to 3 times daily, maximum daily dose of 30 ml (30 mg).
Elderly
No specific dose adjustment is required, but use with caution due to potential for reduced renal function.
Renal_impairment
Dose frequency should be reduced to once or twice daily depending on the severity of impairment. Daily dose should not exceed 10 mg (10 ml).
How to Take
Administer orally, preferably 15-30 minutes before meals. Shake well before use.
Mechanism of Action
Domperidone selectively blocks dopamine D2 and D3 receptors in the chemoreceptor trigger zone (CTZ) and peripherally in the gastrointestinal tract. This action increases esophageal peristalsis, lower esophageal sphincter pressure, gastric emptying, and small bowel peristalsis, thus producing its prokinetic and antiemetic effects.

Absorption
Rapidly absorbed after oral administration; however, absolute bioavailability is low (approximately 15%) due to extensive first-pass metabolism in the gut wall and liver.
Metabolism
Extensive hepatic metabolism, primarily by CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to domperidone or any excipients
- Prolactin-releasing pituitary tumor (prolactinoma)
- When stimulation of gastric motility is dangerous (e.g., in gastrointestinal hemorrhage, mechanical obstruction or perforation)
- Patients with moderate or severe hepatic impairment
- Known existing prolongation of cardiac conduction intervals, particularly QTc, in patients with significant electrolyte disturbances or underlying cardiac diseases such as congestive heart failure
- Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin) or other drugs known to prolong the QTc interval
Drug Interactions
Antacids and H2-receptor antagonists
Can reduce the bioavailability of domperidone. Should be taken after domperidone.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
Can increase domperidone plasma levels, increasing the risk of QTc prolongation. Concomitant use is contraindicated.
QTc-prolonging drugs (e.g., certain antipsychotics, antidepressants, antiarrhythmics)
Increased risk of QTc prolongation. Concomitant use should be avoided or used with extreme caution.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, disorientation, and extrapyramidal reactions, particularly in children. Management is supportive and symptomatic, including gastric lavage, administration of activated charcoal, and close monitoring of cardiac function.

Pregnancy Category C. Domperidone is excreted in breast milk and may cause adverse effects in the infant. Use during pregnancy and lactation only if clearly needed and the potential benefits outweigh the risks.
