Description
Overview of the medicine
Glicana 100 mg Tablet contains Canagliflozin, an SGLT2 inhibitor used to lower blood sugar levels in adults with type 2 diabetes mellitus. It also helps reduce the risk of cardiovascular events, hospitalization for heart failure, and progression of kidney disease in certain patient populations.
Uses & Indications
Dosage
Adults
Initial dose is 100 mg once daily, taken before the first meal of the day. May be increased to 300 mg once daily if additional glycemic control is needed and tolerated.
Elderly
No specific dose adjustment is required based on age alone. Renal function should be assessed before initiation and periodically thereafter.
Renal_impairment
eGFR 45 to <60 mL/min/1.73m²: 100 mg once daily. eGFR <45 mL/min/1.73m²: Not recommended for glycemic control; if initiated for cardiovascular/renal benefits, caution is advised. Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²) for glycemic control or in patients on dialysis.
How to Take
Take Glicana 100 mg Tablet orally once daily, before the first meal of the day. Swallow the tablet whole with water. Do not crush, break, or chew it.
Mechanism of Action
Canagliflozin works by inhibiting the sodium-glucose cotransporter 2 (SGLT2) in the kidneys, which is responsible for reabsorbing glucose back into the bloodstream. By blocking SGLT2, Canagliflozin causes excess glucose to be excreted in the urine, thereby lowering blood sugar levels.
Pharmacokinetics
Onset
Within hours (glucose-lowering effect)
Excretion
Approximately 40% excreted in urine (as metabolites and unchanged drug) and 60% in feces (mainly unchanged drug).
Half life
Approximately 10-13 hours
Absorption
Rapidly absorbed, peak plasma concentrations typically occur 1-2 hours post-dose. Bioavailability is approximately 65%.
Metabolism
Primarily metabolized via O-glucuronidation by UGT1A2, UGT1A3, UGT1A9, and UGT2B4 to inactive O-glucuronide metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Canagliflozin or any excipients.
- •Severe renal impairment (eGFR <30 mL/min/1.73m²) for glycemic control or patients on dialysis.
Drug Interactions
Digoxin
May increase digoxin exposure; monitor digoxin levels.
Diuretics
May increase the risk of dehydration and hypotension.
Rifampin (and other UGT enzyme inducers)
May decrease Canagliflozin exposure; consider increasing Canagliflozin dose.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive treatment should be initiated. Canagliflozin is minimally dialyzable.
Pregnancy & Lactation
Not recommended during pregnancy due to potential fetal renal dysfunction. Use during lactation is also not recommended, as Canagliflozin is excreted in animal milk and potential for serious adverse reactions in breastfed infants is unknown.
