Description
Overview of the medicine
Glifomet is an oral antidiabetic medicine containing Metformin Hydrochloride. It is used to treat type 2 diabetes mellitus, especially in overweight patients, when diet and exercise alone do not provide adequate glycemic control. It can be used alone or in combination with other antidiabetic agents or insulin.
Uses & Indications
Dosage
Adults
Initial dose 500 mg once or twice daily with meals. Gradually increase to a maximum of 2000-2550 mg daily in divided doses. Extended-release formulations may be taken once daily.
Elderly
Initiate with lower doses and titrate slowly. Renal function should be closely monitored.
Renal_impairment
Contraindicated if eGFR <30 mL/min. Dose adjustment is required for eGFR 30-60 mL/min, with careful monitoring.
How to Take
Take with meals to reduce gastrointestinal side effects. Swallow tablets whole; do not crush, chew, or break extended-release tablets.
Mechanism of Action
Metformin works by decreasing hepatic glucose production (gluconeogenesis), decreasing intestinal absorption of glucose, and by improving insulin sensitivity (increasing peripheral glucose uptake and utilization).

Onset
Onset of action is within hours, but full glycemic effect may take days to weeks.
Excretion
Excreted unchanged in the urine, primarily via renal excretion.
Half life
Plasma elimination half-life is approximately 6.2 hours.
Absorption
Oral bioavailability is 50-60%. Food decreases the extent and slightly delays the absorption.
Metabolism
Metformin is not metabolized in the liver.
Side Effects
Contraindications
- •Severe renal impairment (eGFR <30 mL/min/1.73 m²)
- •Metabolic acidosis (including diabetic ketoacidosis)
- •Hypersensitivity to metformin or any component of the formulation
Drug Interactions
Iodinated Contrast Agents
Concomitant use may increase the risk of lactic acidosis. Discontinue metformin at the time of or prior to the imaging procedure and restart 48 hours after, provided renal function is stable.
Diuretics (e.g., Thiazides, Loop Diuretics)
May increase or decrease blood glucose levels, requiring adjustment of antidiabetic therapy. Also, may increase risk of lactic acidosis by impairing renal function.
Cimetidine, Ranolazine, Dolutegravir, Trimethoprim, Vandetanib, Isavuconazole
May increase metformin plasma concentrations by inhibiting renal tubular secretion, increasing the risk of lactic acidosis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Metformin overdose can lead to lactic acidosis, which is a medical emergency. Treatment involves prompt hemodialysis to remove metformin and correct acidosis, along with supportive care.
Pregnancy & Lactation
Pregnancy Category B. Limited data suggest metformin may be used if insulin is not feasible. Metformin is excreted in breast milk; however, amounts are generally considered too small to cause harm in breastfed infants. Use only if clearly needed and benefits outweigh risks.
