Overview of the medicine
Glipizide is an oral sulfonylurea antidiabetic drug used to manage type 2 diabetes mellitus. It helps to lower blood glucose levels by stimulating insulin release from the pancreatic beta cells.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, 30 minutes before breakfast. May be adjusted gradually based on blood glucose response, usually 2.5-20 mg daily in single or divided doses. Maximum recommended daily dose is 40 mg.
Elderly
Lower initial dose (e.g., 2.5 mg once daily) and slower titration recommended due to increased sensitivity and risk of hypoglycemia.
Renal_impairment
Initial dose 2.5 mg once daily. Dose adjustment may be necessary based on renal function and glycemic control. Avoid in severe renal impairment.
How to Take
Take orally, typically 30 minutes before the first meal of the day. If taking multiple doses, take before meals.
Mechanism of Action
Glipizide binds to the sulfonylurea receptor on the pancreatic beta cell membrane, leading to closure of ATP-sensitive potassium channels. This causes depolarization of the cell membrane, opening of voltage-dependent calcium channels, and subsequent influx of calcium ions, which stimulates insulin secretion.

Absorption

Side Effects
Contraindications
- •Hypersensitivity to glipizide or other sulfonylureas.
- •Type 1 diabetes mellitus.
- •Diabetic ketoacidosis (with or without coma).
- •Severe renal or hepatic impairment.
- •Concomitant use with bosentan.
Drug Interactions
Miconazole (oral)
Potentiates the hypoglycemic effect; concomitant use is contraindicated.
Beta-blockers, alcohol, NSAIDs, salicylates, sulfonamides, chloramphenicol, probenecid, coumarins, monoamine oxidase inhibitors, and others
May enhance the hypoglycemic effect of glipizide, increasing the risk of hypoglycemia.
Thiazides and other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, calcium channel blockers, and isoniazid
May reduce the hypoglycemic effect of glipizide, leading to hyperglycemia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypoglycemia (nervousness, sweating, tremor, hunger, confusion, collapse, coma). Management involves administering oral glucose if conscious, or intravenous glucose/glucagon if unconscious. Monitor blood glucose for at least 24-48 hours.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy, especially near term, due to potential fetal hypoglycemia. Use only if potential benefit justifies the potential risk to the fetus. Avoid during breastfeeding due to the potential for hypoglycemia in the infant.
