Description
Overview of the medicine
Glisulin 2 mg Tablet is an oral antidiabetic medicine used to lower blood glucose levels in patients with type 2 diabetes mellitus. It belongs to the sulfonylurea class of drugs, which work by stimulating insulin release from the pancreatic beta cells.
Uses & Indications
Dosage
Adults
Initially 1-2 mg once daily, preferably with breakfast or the first main meal. Dosage may be adjusted in increments of 1 mg at weekly intervals based on blood glucose response, up to a maximum of 20 mg/day.
Elderly
Lower starting dose (e.g., 1 mg once daily) and slower titration recommended due to increased risk of hypoglycemia and renal impairment.
Renal_impairment
Caution advised; lower doses and close monitoring of blood glucose and renal function are necessary. Not recommended for severe renal impairment.
How to Take
Take orally once daily, preferably with breakfast or the first main meal. Swallow the tablet whole with water. Do not crush, chew, or break it.
Mechanism of Action
Glisulin acts by binding to the sulfonylurea receptor (SUR1) on the pancreatic beta-cell membrane, leading to the closure of ATP-sensitive potassium channels. This causes depolarization of the beta-cell membrane, opening of voltage-gated calcium channels, and subsequent influx of calcium. The increased intracellular calcium triggers insulin secretion from the beta cells.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted as metabolites in urine (60%) and feces (40%).
Half life
Approximately 10-16 hours
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are usually reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes (e.g., CYP2C9).
Side Effects
Contraindications
- Hypersensitivity to Glisulin or any component of the formulation
- Type 1 Diabetes Mellitus
- Diabetic ketoacidosis
- Severe renal or hepatic impairment
- Pregnancy and lactation
Drug Interactions
NSAIDs
May enhance the hypoglycemic effect of sulfonylureas.
Fluconazole
Increases Glisulin's plasma concentrations, leading to increased risk of hypoglycemia.
Beta-blockers
May mask symptoms of hypoglycemia and potentiate the hypoglycemic effect.
Thiazide diuretics
May reduce the hypoglycemic effect of sulfonylureas.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypoglycemia. Treatment involves administration of glucose (oral or intravenous), close monitoring of blood glucose levels, and supportive care. If conscious, oral glucose or sugary drinks should be given. In severe cases, intravenous dextrose may be required.
Pregnancy & Lactation
Glisulin is generally not recommended during pregnancy and lactation. Insulin therapy is usually preferred for glycemic control in pregnant women. If taken during lactation, there is a risk of hypoglycemia in the infant.

YesClinical Trials
Various clinical trials have demonstrated the efficacy and safety of sulfonylureas in managing type 2 diabetes, showing significant reductions in HbA1c levels. Long-term studies have also explored cardiovascular outcomes.
Lab Monitoring
- Fasting plasma glucose (FPG)
- HbA1c levels
- Renal function tests (e.g., serum creatinine, eGFR)
- Liver function tests (e.g., ALT, AST)
Doctor Notes
- Emphasize patient education on hypoglycemia recognition and management.
- Regular monitoring of blood glucose, HbA1c, renal, and hepatic function is crucial.
- Consider alternative therapies if hypoglycemia is a persistent issue or in patients with significant comorbidities.
Patient Guidelines
- Take medicine exactly as prescribed by your doctor.
- Monitor your blood glucose levels regularly as advised.
- Carry a source of sugar (e.g., glucose tablets, candy) with you to treat mild hypoglycemia.
- Do not stop taking Glisulin without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Patients should be aware of the symptoms of hypoglycemia (e.g., dizziness, blurred vision, confusion) which may impair their ability to drive or operate machinery. If these symptoms occur, avoid driving.
Lifestyle Advice
- Maintain a balanced diet appropriate for diabetes.
- Engage in regular physical activity.
- Avoid excessive alcohol consumption.
- Manage stress effectively.
