Description
Overview of the medicine
Sertraline is an antidepressant of the selective serotonin reuptake inhibitor (SSRI) class. It is used to treat major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).
Uses & Indications
Dosage
Adults
Typical starting dose for most indications is 50 mg orally once daily. Dosage may be increased gradually, typically at weekly intervals, up to a maximum of 200 mg/day depending on response and tolerability.
Elderly
Similar to adult dosage, but caution should be exercised and lower starting doses or slower titration may be considered due to potential for decreased clearance and increased sensitivity.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take orally once daily, in the morning or evening, with or without food. Swallow the capsule whole; do not chew, crush, or open.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Therapeutic effects may take 2-4 weeks to become apparent, with full effect potentially taking longer.
Excretion
Excreted in urine and feces as metabolites. Less than 2% is excreted unchanged in urine.
Half life
Approximately 26 hours (range 22-36 hours) for sertraline and 62-104 hours for the active metabolite desmethylsertraline.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached 4.5-8.4 hours after dosing. Food slightly increases absorption and reduces the time to reach peak plasma levels.
Metabolism
Extensive hepatic metabolism, primarily via CYP2B6, CYP2C19, CYP2C9, CYP3A4, and CYP2D6 pathways, to inactive and one active (desmethylsertraline) metabolite.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of stopping MAOI treatment
- Concomitant use with Pimozide
- Hypersensitivity to sertraline or any component of the formulation
Drug Interactions
MAOIs
Serious, sometimes fatal, reactions including Serotonin Syndrome.
NSAIDs
Increased risk of gastrointestinal bleeding.
Pimozide
Increased pimozide levels and cardiotoxicity.
Triptans
Increased risk of Serotonin Syndrome.
Warfarin
Potentiation of anticoagulant effect, increased bleeding risk.
Other Serotonergic Drugs (e.g., other SSRIs, SNRIs, Fentanyl, Lithium, Tramadol, St. John's Wort)
Increased risk of Serotonin Syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, convulsions and coma. Management is supportive and symptomatic; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Sertraline is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing woman.
