Overview of the medicine
Glucium 250 mg tablet is an oral anti-diabetic medication used to improve glycemic control in adults with type 2 diabetes mellitus. It works by reducing glucose reabsorption in the kidneys, leading to increased glucose excretion in the urine.
Uses & Indications
Dosage
Adults
Initial dose is 250 mg once daily, taken in the morning with or without food. Maximum dose 250 mg once daily.
Elderly
No dose adjustment generally required based on age, but renal function should be monitored.
Renal_impairment
Not recommended for initiation in patients with eGFR < 30 mL/min/1.73m². Dose adjustment may be necessary based on eGFR levels.
How to Take
Take orally once daily in the morning, with or without food.
Mechanism of Action
Glucium selectively inhibits sodium-glucose co-transporter 2 (SGLT2) in the renal proximal tubules, reducing glucose reabsorption and lowering the renal glucose threshold, thereby increasing urinary glucose excretion and lowering blood glucose levels.

Onset
Within hours of administration, significant glucosuric effect observed.
Excretion
Mainly excreted renally as metabolites, with a small portion excreted unchanged in urine.
Half life
Approximately 10-12 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via glucuronidation.
Side Effects
Contraindications
- •History of serious hypersensitivity reaction to Glucium or any of its excipients.
- •Patients with end-stage renal disease or on dialysis.
Drug Interactions
Lithium
Decreased serum lithium concentrations may occur due to increased renal lithium excretion.
Diuretics
May increase the risk of dehydration and hypotension.
Insulin or Insulin Secretagogues
Increased risk of hypoglycemia, dose adjustment may be necessary.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
In case of an overdose, contact a poison control center or emergency room immediately. Treatment should be supportive, including monitoring of vital signs and clinical status. Removal of unabsorbed drug via gastric lavage or activated charcoal may be considered.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the second and third trimesters, due to potential adverse effects on fetal renal development. Use during lactation is not recommended as it is unknown if Glucium is excreted in human milk.
