Description
Overview of the medicine
Gliclazide is an oral hypoglycemic agent from the sulfonylurea class, used to manage type 2 diabetes mellitus when diet, exercise, and weight reduction alone are not sufficient.
Uses & Indications
Dosage
Adults
Initial dose 40-80 mg once daily, taken with breakfast. May be adjusted up to 320 mg/day in divided doses, based on glycemic response.
Elderly
Lower initial dose (e.g., 40 mg) and careful titration due to increased sensitivity to hypoglycemic effects and potential for renal impairment.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment, but caution is advised. Not recommended for severe impairment.
How to Take
Orally, once daily with breakfast. Swallow whole with water; do not crush, chew, or break the tablet.
Mechanism of Action
Stimulates insulin release from pancreatic beta cells, reduces hepatic glucose production, and increases peripheral glucose utilization.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted in urine (less than 1% as unchanged drug), some via feces.
Half life
Approximately 10-12 hours for the conventional release tablet.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are typically reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C9, forming inactive metabolites.
Side Effects
Contraindications
- Type 1 Diabetes Mellitus
- Diabetic ketoacidosis, diabetic pre-coma, and coma
- Severe renal or hepatic impairment
- Hypersensitivity to gliclazide, other sulfonylureas, or sulfonamides
- Pregnancy and lactation
Drug Interactions
Fluconazole
Increases plasma concentration of gliclazide, enhancing hypoglycemic effect.
Beta-blockers
May mask symptoms of hypoglycemia and increase hypoglycemic effect.
Corticosteroids
May reduce the hypoglycemic effect of gliclazide (hyperglycemic effect).
NSAIDs (e.g., Phenylbutazone)
Potentiate the hypoglycemic effect of sulfonylureas, increasing risk of hypoglycemia.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose can lead to severe hypoglycemia, potentially requiring hospital admission. Management involves oral glucose or intravenous dextrose, and close monitoring of blood glucose.
Pregnancy & Lactation
Not recommended during pregnancy and breastfeeding. Insulin is the preferred treatment for glycemic control in these situations.

Lab Monitoring
- Blood glucose levels (fasting and postprandial)
- HbA1c (glycated hemoglobin)
- Liver function tests (LFTs)
- Renal function tests (RFTs)
Doctor Notes
- Emphasize comprehensive patient education on hypoglycemia recognition and management, including carrying readily available sugar sources.
- Regular monitoring of renal and hepatic function is important, particularly in elderly patients or those with pre-existing conditions.
- Carefully assess and consider all risk factors for hypoglycemia before initiating and adjusting gliclazide therapy.
Patient Guidelines
- Take Glucoact-80 mg exactly as prescribed by your doctor, usually with breakfast.
- Monitor your blood sugar levels regularly as advised.
- Be aware of the symptoms of hypoglycemia (e.g., sweating, trembling, confusion) and carry a source of sugar (e.g., glucose tablets, fruit juice).
- Do not skip meals while taking this medication.
Missed Dose Advice
If a dose is missed, take it as soon as you remember unless it's almost time for the next dose. Do not take a double dose to make up for a forgotten dose.
Driving Precautions
Glucoact-80 mg may cause hypoglycemia, which can impair your ability to concentrate and react. Patients should be aware of their response to the medication before driving or operating machinery.
Lifestyle Advice
- Follow a balanced diet for diabetes, focusing on low glycemic index foods.
- Engage in regular physical activity suitable for your health condition.
- Maintain a healthy weight through diet and exercise.
- Avoid excessive alcohol consumption as it can increase the risk of hypoglycemia.
