Description
Overview of the medicine
GlucoHart Injection is a novel medication indicated for the management of type 2 diabetes mellitus and reduction of major adverse cardiovascular events in adults with established cardiovascular disease or multiple cardiovascular risk factors.
Uses & Indications
Dosage
Adults
Initial dose 5 mg subcutaneously once daily, may be titrated up to 20 mg once daily based on glycemic control and tolerability.
Elderly
No dose adjustment typically required based on age alone; monitor renal function.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (eGFR <30 mL/min/1.73m²).
How to Take
Subcutaneous injection in the abdomen, thigh, or upper arm. Rotate injection sites. Can be administered at any time of day, with or without meals.
Mechanism of Action
GlucoHart Injection acts through a dual mechanism by enhancing insulin sensitivity in peripheral tissues and reducing hepatic glucose output, while also exerting direct cardioprotective effects through modulation of inflammatory pathways and improvement of endothelial function.
Pharmacokinetics
Onset
Glucose-lowering effects observed within hours; cardiovascular benefits manifest over weeks to months.
Excretion
Excreted predominantly via renal clearance of inactive metabolites.
Half life
Approximately 18-24 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed after subcutaneous injection, reaching peak plasma concentrations within 1-2 hours.
Metabolism
Primarily metabolized by peptidases; minimal hepatic cytochrome P450 involvement.
Side Effects
Contraindications
- Hypersensitivity to the active substance or excipients
- Type 1 Diabetes Mellitus
- Diabetic ketoacidosis
Drug Interactions
Insulin or Sulfonylureas
Increased risk of hypoglycemia, dose adjustment may be required.
Other glucose-lowering agents
Monitor for additive glycemic effects.
Storage
Store in a refrigerator (2°C to 8°C). Do not freeze. Protect from light. After first use, store at room temperature (below 30°C) or in a refrigerator (2°C to 8°C).
Overdose
Symptoms of overdose may include severe hypoglycemia. Management involves supportive care and correction of hypoglycemia, potentially with intravenous glucose.
Pregnancy & Lactation
Use during pregnancy is not recommended unless the potential benefit justifies the potential risk to the fetus. It is unknown whether GlucoHart is excreted in human milk; caution should be exercised when administered to a nursing woman.
