Description
Overview of the medicine
Glycema 5 mg Tablet is an oral antidiabetic medication used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing the excretion of glucose in the urine.
Uses & Indications
Dosage
Adults
The recommended starting dose is 5 mg orally once daily in the morning, with or without food. Dosage may be increased to 10 mg once daily if additional glycemic control is needed and tolerated.
Elderly
No dose adjustment is required based on age alone, but renal function should be monitored.
Renal_impairment
Not recommended for patients with eGFR persistently below 30 mL/min/1.73m². Dose adjustment may be needed for moderate impairment; consult prescribing information.
How to Take
Take Glycema 5 mg Tablet orally once daily in the morning, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Glycema, as an SGLT2 inhibitor, selectively inhibits the sodium-glucose cotransporter 2 (SGLT2) in the renal proximal tubules. This reduces glucose reabsorption from the glomerular filtrate and increases urinary glucose excretion, leading to a reduction in blood glucose levels.
Pharmacokinetics
Onset
Glucose-lowering effects observed within hours of the first dose.
Excretion
Primarily excreted via urine (approximately two-thirds) and feces (one-third) as metabolites and unchanged drug.
Half life
Approximately 10-13 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1-2 hours. Bioavailability is high.
Metabolism
Primarily metabolized in the liver via glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Patients with severe renal impairment (eGFR < 30 mL/min/1.73m²), end-stage renal disease, or on dialysis.
Drug Interactions
Diuretics
May increase the risk of dehydration and hypotension due to increased diuresis. Monitor fluid status and blood pressure.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
May increase the risk of hypoglycemia. A lower dose of insulin or sulfonylurea may be required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Glycema is not expected to be dialyzable.
Pregnancy & Lactation
Not recommended during pregnancy and breastfeeding. Advise women of childbearing potential to use effective contraception. Consult your doctor for alternative treatments if pregnant or planning to conceive.

Lab Monitoring
- Renal function (e.g., eGFR, serum creatinine) should be assessed before initiating treatment and periodically thereafter.
- Blood glucose levels and HbA1c should be monitored regularly.
- Lipid profile and electrolytes.
Doctor Notes
- Emphasize regular monitoring of renal function and electrolyte balance, especially in elderly patients or those on concomitant diuretics.
- Educate patients on symptoms of urinary tract and genital infections and diabetic ketoacidosis, advising prompt medical attention if these occur.
- Consider potential for volume depletion and hypotension, particularly at treatment initiation or with dose escalation.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Stay well hydrated by drinking plenty of fluids.
- Monitor your blood glucose levels regularly as advised by your doctor.
- Be aware of symptoms of genital or urinary tract infections and report them to your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Glycema has no or negligible influence on the ability to drive and use machines. However, patients should be advised on the risk of hypoglycemia when Glycema is used in combination with insulin or sulfonylureas.
Lifestyle Advice
- Maintain a balanced diet and regular exercise routine.
- Manage weight through healthy eating and physical activity.
- Avoid excessive alcohol consumption.
