Description
Overview of the medicine
Gvsil 500 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, and ear infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections, 200 mg once daily or 100 mg twice daily. For severe infections or specific indications like uncomplicated gonorrhea, a single dose of 400 mg or 500 mg once daily may be recommended. The 500 mg sachet is for reconstitution to be taken as directed by a physician.
Elderly
Similar to adult dosage, but caution and dosage adjustment may be needed in cases of significant renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, 75% of the standard dose should be given. For creatinine clearance <20 mL/min, 50% of the standard dose should be given. Consult a physician.
How to Take
For oral use. The powder in the sachet should be reconstituted with the specified amount of water, shaken well, and taken immediately or as directed by the physician. It can be taken with or without food.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers that are essential for bacterial cell wall strength and rigidity, leading to cell lysis and death.

Onset
Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%) over 24 hours.
Half life
Plasma half-life is typically 3-4 hours, but can be prolonged in renal impairment.
Absorption
Approximately 40-50% orally absorbed, absorption may be slightly delayed by food but total amount absorbed remains unchanged.
Metabolism
Cefixime is not extensively metabolized in the body. Some metabolites may be formed but their clinical significance is minimal.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to any penicillin antibiotic.
Drug Interactions
Nifedipine
May increase the bioavailability of cefixime.
Probenecid
Increases peak plasma levels and AUC of cefixime by reducing renal excretion.
Carbamazepine
Increased plasma levels of carbamazepine may occur. Monitor carbamazepine levels.
Oral anticoagulants (e.g., Warfarin)
Increased prothrombin time and bleeding risk. Close monitoring of INR is recommended.
Storage
Store unopened powder sachets below 30°C, protected from light and moisture. Keep out of reach of children. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
