Overview of the medicine
Gynoheal Vaginal Suppository is a combination medication containing an antibacterial (Clindamycin) and an antifungal (Clotrimazole) used for the local treatment of mixed vaginal infections such as bacterial vaginosis and vulvovaginal candidiasis.
Uses & Indications
Dosage
Adults
Insert one Gynoheal Vaginal Suppository deeply into the vagina, preferably at bedtime, for 3 or 7 consecutive days, as directed by a physician.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal.
How to Take
For vaginal use only. Wash hands thoroughly before and after use. Lie on your back with knees bent and gently insert the suppository deep into the vagina using your finger or the provided applicator.
Mechanism of Action
Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. Clotrimazole is an azole antifungal that inhibits fungal cytochrome P450, thereby impairing ergosterol synthesis, an essential component of the fungal cell membrane, leading to increased membrane permeability and fungal cell death.
Pharmacokinetics
Onset
Symptomatic relief usually within 2-3 days, full therapeutic effect within 7 days.
Excretion
Clindamycin and its metabolites are excreted via urine and feces. Clotrimazole is mainly excreted in feces as inactive metabolites.
Half life
Clindamycin (vaginal): Approximately 1.5-2.6 hours (systemic). Clotrimazole (vaginal): Not well characterized due to minimal systemic absorption.
Absorption
Clindamycin is minimally absorbed systemically from the vaginal route (approximately 4-10%). Clotrimazole is also poorly absorbed systemically from the vaginal route (3-10% of administered dose).
Metabolism
Clindamycin is primarily metabolized in the liver to inactive metabolites. Clotrimazole is metabolized in the liver by cytochrome P450 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Clindamycin, Clotrimazole, or any component of the suppository
- •History of regional enteritis, ulcerative colitis, or antibiotic-associated colitis
- •First trimester of pregnancy (relative contraindication for Clindamycin)
Drug Interactions
Erythromycin
Antagonism between Clindamycin and Erythromycin has been demonstrated in vitro.
Neuromuscular blocking agents
Clindamycin has neuromuscular blocking properties which may enhance the action of other neuromuscular blocking agents.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Systemic overdose is unlikely due to minimal vaginal absorption. In case of accidental oral ingestion, supportive and symptomatic treatment should be provided.
Pregnancy & Lactation
Clindamycin vaginal preparations may be used during the second and third trimesters of pregnancy if clearly needed. Clotrimazole vaginal preparations are generally considered safe during pregnancy. Consult your doctor before use. Use with caution during lactation as Clindamycin is excreted in breast milk.
