Description
Overview of the medicine
Hydroxychloroquine is an antimalarial and anti-inflammatory drug used to prevent and treat malaria, and to treat autoimmune diseases like lupus erythematosus and rheumatoid arthritis.
Uses & Indications
Dosage
Adults
Varies by indication. For rheumatoid arthritis, initially 400 mg daily in divided doses, then 200-400 mg daily. For malaria prophylaxis, 400 mg once weekly.
Elderly
Initiate with lower doses and monitor for adverse effects due to potential decreased renal/hepatic function.
Renal_impairment
Dose reduction may be necessary; caution advised.
How to Take
Administer orally, usually with food or milk to minimize gastrointestinal upset.
Mechanism of Action
Hydroxychloroquine works by interfering with parasitic growth in red blood cells. In autoimmune diseases, it modulates immune responses, possibly by inhibiting antigen presentation, lysosomal function, and cytokine production.
Pharmacokinetics
Onset
For malaria, within hours; for rheumatic conditions, weeks to months.
Excretion
Primarily via urine, with a small amount excreted in feces.
Half life
Long elimination half-life, ranging from 32 to 50 days for the parent drug and metabolites.
Absorption
Rapidly absorbed from the GI tract; bioavailability varies but is generally good. Peak plasma concentrations reached in about 3-4 hours.
Metabolism
Extensively metabolized in the liver to active metabolites (e.g., desethylchloroquine).
Side Effects
Contraindications
- Hypersensitivity to 4-aminoquinoline compounds
- Pre-existing maculopathy of the eye
- Long-term use in children (contraindicated by some authorities)
Drug Interactions

May decrease absorption of hydroxychloroquine.
Mefloquine
Increased risk of seizures.
Other QT prolonging drugs
Increased risk of arrhythmias.
Insulin/Hypoglycemic agents
May enhance the effects of these drugs.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include headache, drowsiness, visual disturbances, cardiovascular collapse, and seizures. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal.
