Description
Overview of the medicine
Halotop 0.05% Lotion contains Halobetasol Propionate, a high-potency topical corticosteroid, used for the relief of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin areas once or twice daily, as directed by a physician. Treatment should be limited to 2 consecutive weeks and total dosage should not exceed 50g per week.
Elderly
No specific dosage adjustment required, but caution advised due to potential for thinner skin and increased absorption.
Renal_impairment
No specific dosage adjustment for topical use; however, systemic exposure should be monitored with prolonged use.
How to Take
For external use only. Apply a thin film to the affected skin area. Rub in gently and completely. Do not use with occlusive dressings unless directed by a physician. Avoid contact with eyes and mucous membranes.
Mechanism of Action
Halobetasol Propionate is a corticosteroid that acts by inducing phospholipase A2 inhibitory proteins, called lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Within hours to days of application.
Excretion
Excreted by the kidneys and in bile.
Half life
Not well established for topical application; systemic half-life of corticosteroids generally varies.
Absorption
Systemic absorption of topical corticosteroids can occur, especially with prolonged use, large surface areas, or occlusive dressings. Absorption varies by site, vehicle, and skin integrity. Higher absorption in inflamed skin.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Halobetasol Propionate or any component of the formulation
- Primary cutaneous viral infections (e.g., herpes simplex, varicella)
- Fungal infections
- Bacterial infections (unless concomitant anti-infective therapy is used)
- Rosacea
- Perioral dermatitis
- Acne vulgaris

Drug Interactions
Immunosuppressants
May increase the risk of infection.
Other topical preparations
Concurrent use of other topical preparations on the same area should be avoided unless medically advised to prevent altered absorption or irritation.
Storage
Store at controlled room temperature (20°C to 25°C). Protect from freezing and excessive heat. Keep the tube tightly closed when not in use. Do not refrigerate.
Overdose
Acute overdose with topical corticosteroids is rare. However, prolonged or excessive use may lead to systemic effects such as HPA axis suppression, Cushing's syndrome, hyperglycemia, and glucosuria. Treatment is symptomatic and supportive; gradual withdrawal of the drug may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution is advised when administered to a nursing woman due to potential systemic absorption.

- Emphasize the high potency of Halobetasol Propionate and the importance of limiting treatment duration to avoid adverse effects.
- Advise patients against using on face, groin, or axillae unless specifically instructed.
