Description
Overview of the medicine
Haloxa 0.05% Ointment is a high-potency topical corticosteroid used for the relief of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily, as directed by a physician. Treatment should be limited to 2 weeks, and the total dose should not exceed 50g per week.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential for thinner skin and increased absorption.
Renal_impairment
No specific dosage adjustment provided, but systemic absorption is minimal with proper topical use.
How to Take
For external use only. Apply a thin film to the affected skin area and gently rub it in. Wash hands thoroughly before and after application. Avoid contact with eyes, mouth, and mucous membranes.
Mechanism of Action
Halobetasol propionate is a corticosteroid that acts by inducing phospholipase A2 inhibitory proteins, called lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Within hours of application.
Excretion
Excreted by the kidneys, primarily as inactive metabolites.
Half life
Not well-defined for topical application; systemic half-life of corticosteroids generally ranges from 18-36 hours.
Absorption
Systemic absorption of topical corticosteroids can occur, especially with prolonged use, application to large surface areas, or under occlusive dressings. Absorption is higher in areas with thin skin (e.g., face, groin).
Metabolism
Primarily metabolized in the liver to inactive metabolites. Any absorbed drug is metabolized similarly to systemically administered corticosteroids.
Side Effects
Contraindications
- Hypersensitivity to halobetasol propionate or any component of the formulation
- Bacterial, fungal, viral (e.g., herpes simplex, varicella), or parasitic infections of the skin
- Rosacea
- Perioral dermatitis
- Acne vulgaris
- Perianal and genital pruritus
Drug Interactions
Immunosuppressants
May enhance the immunosuppressive effect, though clinically significant interactions are rare for topical preparations.
Systemic Corticosteroids
Increased risk of systemic side effects if used concomitantly, especially with extensive application or occlusive dressings.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Excessive or prolonged use of topical corticosteroids can suppress the pituitary-adrenal axis, resulting in secondary adrenal insufficiency. Symptoms may include Cushing's syndrome, hyperglycemia, and glucosuria. Treatment is symptomatic and supportive; gradual withdrawal of the drug may be necessary.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Topical corticosteroids should not be used extensively or for prolonged periods during pregnancy. Caution should be exercised when administered to a nursing mother. Consult a doctor.

Lab Monitoring
- Periodic evaluation for hypothalamic-pituitary-adrenal (HPA) axis suppression may be warranted for patients applying topical steroids to a large surface area or under occlusion.
Doctor Notes
- Emphasize limited duration of use (max 2 weeks) and quantity (max 50g/week) to minimize systemic absorption and local side effects.
- Advise patients against using on sensitive areas like the face or groin unless absolutely necessary and under strict supervision.
- Educate on signs of skin atrophy and adrenal suppression, and when to seek medical attention.
Patient Guidelines
- Use exactly as prescribed by your doctor.
- Do not cover the treated area with bandages or other dressings unless directed by your doctor.
- Avoid using on the face, groin, or axillae unless specifically instructed.
- Do not use for longer than 2 weeks without consulting your doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
Haloxa 0.05% Ointment is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Keep the affected skin clean and dry.
- Avoid harsh soaps and irritants that may worsen the skin condition.
