Description
Overview of the medicine
Hema 50 mg Capsule is an iron supplement used to prevent and treat iron deficiency anemia. It helps in the formation of red blood cells and supports overall health.
Uses & Indications
Dosage
Adults
Typically 50-100 mg elemental iron daily, or as directed by physician. May be taken once or twice daily.
Elderly
Same as adult dose, but caution with gastrointestinal side effects.
Renal_impairment
No specific dose adjustment for elemental iron, but monitor iron levels. Avoid in non-dialysis dependent CKD without iron deficiency due to risk of iron overload.
How to Take
Take with water, preferably on an empty stomach for maximum absorption, or with food to minimize gastrointestinal upset. Do not take with antacids, dairy products, or tea/coffee as they can reduce absorption. Vitamin C can enhance absorption.
Mechanism of Action
Iron is an essential mineral primarily required for erythropoiesis (red blood cell formation). It is a crucial component of hemoglobin, which transports oxygen from the lungs to all body tissues. Iron also plays a vital role in various metabolic processes, including energy production and DNA synthesis.
Pharmacokinetics
Onset
Improvement in symptoms may be seen within a few weeks, but full recovery of iron stores takes several months.
Excretion
Very little iron is excreted. Small amounts are lost through shedding of skin/mucosal cells, sweat, urine, and feces.
Half life
Not applicable for elemental iron as it's metabolized and utilized, not eliminated rapidly as a drug. Red blood cell half-life is ~120 days.
Absorption
Primarily absorbed in the duodenum and upper jejunum, absorption is enhanced in iron-deficient states.
Metabolism
Iron is primarily incorporated into hemoglobin, myoglobin, and iron-containing enzymes. Excess iron is stored as ferritin or hemosiderin.
Side Effects
Contraindications
- •Hemochromatosis or other iron overload syndromes.
- •Non-iron deficiency anemia (e.g., hemolytic anemia, thalassemia major).
- •Hypersensitivity to iron preparations.
Drug Interactions
Levothyroxine
Iron can reduce levothyroxine absorption. Separate administration by at least 4 hours.
Tetracyclines, quinolones
Iron can reduce absorption of these antibiotics. Separate administration by at least 2 hours.
Antacids, proton pump inhibitors, H2 blockers
Decrease iron absorption.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include severe nausea, vomiting, abdominal pain, diarrhea, lethargy, and sometimes shock or coma. Severe cases can lead to metabolic acidosis, liver damage, and death. Seek immediate medical attention. Treatment involves supportive care, gastric lavage, and chelation therapy with deferoxamine in severe cases.
Pregnancy & Lactation
Generally considered safe and often recommended during pregnancy and lactation to prevent and treat iron deficiency anemia. Consult a doctor before use.
