Description
Overview of the medicine
Hemadus 50 mg Tablet contains Hemadusan, an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The usual starting dose is 50 mg once daily. The dose may be increased to 100 mg once daily if necessary, based on blood pressure response.
Elderly
No initial dose adjustment is generally required, but consider starting with a lower dose (e.g., 25 mg) in some elderly patients.
Renal_impairment
No initial dose adjustment is required for patients with mild to moderate renal impairment. For severe impairment or dialysis, consult a physician.
How to Take
Hemadus tablets should be taken orally, once daily, with or without food. It is recommended to take the tablet at about the same time each day.
Mechanism of Action
Hemadusan selectively blocks the binding of Angiotensin II to the AT1 receptor found in many tissues, including vascular smooth muscle and the adrenal gland. This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of Angiotensin II, leading to vasodilation, decreased blood pressure, and reduced sodium and water retention.
Pharmacokinetics
Onset
Antihypertensive effect usually observed within 1-2 hours, maximum effect within 3-6 weeks.
Excretion
Excreted via urine (primarily as metabolite) and feces (primarily as unchanged drug and metabolite).
Half life
Approximately 6-9 hours for Hemadusan; active metabolite has a longer half-life (12-15 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Partially metabolized in the liver (via CYP2C9 and CYP3A4) to an active carboxylic acid metabolite.
Side Effects
Contraindications
- Hypersensitivity to Hemadusan or any excipients
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m²)
Drug Interactions
Lithium
May increase serum lithium concentrations and lead to lithium toxicity.
Other antihypertensives
May have additive hypotensive effects.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics (e.g., spironolactone) and potassium supplements
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure), dizziness, and tachycardia (rapid heartbeat), or bradycardia (slow heartbeat). Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage may be considered. Hemadusan is not removable by hemodialysis.
Pregnancy & Lactation
Hemadus is contraindicated during pregnancy, especially in the second and third trimesters, due to the risk of fetal injury and death. It is not recommended during lactation as it is unknown if Hemadusan is excreted in human milk.

Lab Monitoring
- Serum potassium levels (especially in patients with renal impairment or those taking potassium-sparing diuretics)
- Renal function (e.g., serum creatinine, BUN) periodically
- Liver function tests (if clinically indicated)
Doctor Notes
- Monitor blood pressure regularly and adjust dose as needed.
- Assess renal function and serum potassium before and during therapy, especially in high-risk patients.
- Counsel patients on the importance of adherence and lifestyle modifications.
- Avoid in pregnant patients; switch to alternative antihypertensive therapy.
Patient Guidelines
- Take Hemadus regularly as prescribed, even if you feel well.
- Do not stop taking the medicine without consulting your doctor.
