Description
Overview of the medicine
Hemofer 100 mg Injection contains Iron Sucrose, which is an intravenous iron preparation used for treating iron deficiency anemia in patients undergoing hemodialysis who are receiving erythropoietin therapy, and for other specified cases of iron deficiency where oral iron is ineffective or contraindicated.
Uses & Indications
Dosage
Adults
Typically 100-200 mg (2-4 mL) intravenously 1-3 times per week, total cumulative dose depends on patient's iron deficit.
Elderly
Similar to adult dosage, careful monitoring is advised.
Renal_impairment
Specific dosing adjustment may not be required for hemodialysis patients as per standard protocol, but overall iron status should guide.
How to Take
Administered as a slow intravenous injection or infusion. Must be diluted in 0.9% NaCl solution for infusion. Should not be mixed with other medications or used with other parenteral iron preparations.
Mechanism of Action
Iron sucrose complex is absorbed by the reticuloendothelial system. The iron is then slowly released and transferred to transferrin and subsequently to the bone marrow for hemoglobin synthesis. It replenishes iron stores and improves hemoglobin levels.
Pharmacokinetics
Onset
Hemoglobin levels typically begin to rise within a few weeks.
Excretion
Primarily through the reticuloendothelial system, with minimal renal excretion.
Half life
Terminal elimination half-life is approximately 6 hours.
Absorption
Rapidly distributed to plasma and reticuloendothelial system after IV administration.
Metabolism
The sucrose component is metabolized by glycolysis, while the iron component is utilized in iron synthesis or stored.
Side Effects
Contraindications
- Known hypersensitivity to iron sucrose or any of its components.
- Anemias not caused by iron deficiency (e.g., hemolytic anemia).
- Iron overload (hemochromatosis, hemosiderosis) or disturbances in iron utilization.
Drug Interactions
Oral iron preparations
Concomitant administration with oral iron preparations may reduce the absorption of oral iron. Oral iron should not be given until at least 5 days after the last iron sucrose injection.
Storage
Store at room temperature (20°C to 25°C), protect from light and freezing. Do not store above 30°C.
Overdose
Overdose may lead to acute iron overload, which can manifest as hypotension, collapse, shock, and potentially severe organ damage. Treatment involves supportive care and iron chelation therapy if severe.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant women have shown no evidence of harm to the fetus. Use during lactation is generally considered safe, but caution is advised. Consult a doctor.
