Description
Overview of the medicine
Hemostat 500 mg Capsule contains Tranexamic Acid, an anti-fibrinolytic agent used to prevent and treat excessive bleeding.
Uses & Indications
Dosage
Adults
General dosage for bleeding: 1-1.5 g (2-3 capsules) orally 2-3 times daily for 3-7 days, depending on the indication. For menorrhagia: 1 g (2 capsules) 3 times daily for up to 4 days during menstruation.
Elderly
Dosage adjustments may be necessary in elderly patients with impaired renal function. Close monitoring is advised.
Renal_impairment
Reduced dosage is required. For mild to moderate impairment, adjust dosage based on creatinine clearance. Not recommended in severe renal impairment without expert consultation.
How to Take
Hemostat 500 mg Capsule should be taken orally, with or without food. Swallow the capsule whole with a glass of water. Do not chew or crush.
Mechanism of Action
Tranexamic acid competitively inhibits the activation of plasminogen to plasmin, thereby preventing the dissolution of fibrin clots and stabilizing existing clots. This leads to a reduction in bleeding.
Pharmacokinetics
Onset
Anti-fibrinolytic effects begin within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration. About 95% of an oral dose is excreted unchanged within 24 hours.
Half life
Approximately 2-11 hours (dose and renal function dependent).
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 30-50%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Minimally metabolized; approximately 5% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to tranexamic acid or any component of the formulation
- Active intravascular clotting
- Severe renal impairment (due to risk of accumulation)
- History of convulsions
- Subarachnoid hemorrhage (risk of cerebral edema and infarction)
Drug Interactions
Oral Contraceptives / Estrogens
Increased risk of thrombosis (blood clots).
Thrombolytic agents (e.g., Alteplase)
Concurrent use is generally contraindicated as tranexamic acid is an antifibrinolytic.
Anticoagulants (e.g., Warfarin, Heparin)
May counteract the effects of anticoagulants, requiring careful monitoring.
Factor IX complex concentrates / Anti-inhibitor coagulant complexes
Potential for increased thrombogenic risk.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, orthostatic hypotension, dizziness, headache, and thrombotic events. Management is symptomatic and supportive, including gastric lavage and activated charcoal if recent ingestion.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Consult a doctor. Lactation: Tranexamic acid is excreted in breast milk. Use with caution in nursing mothers, as the clinical effect on the infant is unknown.

Lab Monitoring
- Regular ophthalmological examinations are recommended for patients on long-term treatment (e.g., for hereditary angioedema) to detect any visual disturbances.
- Renal function tests should be monitored periodically, especially in elderly patients or those with known renal impairment.
Doctor Notes
- Carefully assess the patient's risk factors for thrombosis before initiating tranexamic acid.
- Consider renal function adjustment in elderly patients or those with kidney impairment.
- Advise patients to report any visual changes or symptoms of thrombosis immediately.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking the medicine abruptly without consulting your doctor, even if you feel better.
- Report any unusual bleeding, bruising, or changes in vision immediately to your doctor.
- Inform your doctor or pharmacist about all other medicines, including over-the-counter drugs and herbal supplements, before starting Hemostat.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Hemostat 500 mg Capsule generally does not affect the ability to drive or operate machinery. However, if you experience dizziness or visual disturbances, avoid such activities.
Lifestyle Advice
- Maintain good hydration by drinking plenty of fluids.
- Avoid smoking, as it can increase the risk of blood clots.
