Description
Overview of the medicine
Henxit 0.5 mg Tablet contains Clonazepam, a benzodiazepine, primarily used to treat various seizure disorders including Lennox-Gastaut syndrome, akinetic and myoclonic seizures. It is also used for panic disorder and sometimes for restless legs syndrome.
Uses & Indications
Dosage
Adults
Initial dose for seizure disorders: 0.5 mg three times daily. May be increased by 0.5 mg to 1 mg every 3 days until seizure control is achieved or side effects preclude further increase. Max: 20 mg/day. For panic disorder: Initial 0.25 mg twice daily, increased to 1 mg/day after 3 days. Max: 4 mg/day.
Elderly
Lower initial doses (e.g., 0.25 mg once daily) and slower titration recommended due to increased sensitivity and risk of adverse effects.
Renal_impairment
No specific dosage adjustment recommended for mild to moderate renal impairment, but caution is advised. Not recommended in severe impairment.
How to Take
Take orally with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Clonazepam enhances the effect of gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter in the central nervous system. It binds to benzodiazepine receptors on GABA-A receptor complexes, which leads to an increased influx of chloride ions into the neuron, causing hyperpolarization and reducing neuronal excitability.

Excretion
Excreted mainly in the urine as metabolites, with a small amount excreted unchanged.
Half life
Plasma elimination half-life is 18-50 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by nitroreduction, to 7-amino-clonazepam, which is then acetylated. CYP3A4 is involved.
