Overview of the medicine
Heparon (Heparin Sodium) is an anticoagulant medication used to prevent the formation of blood clots and to treat existing clots. It works by enhancing the activity of antithrombin III, an enzyme that inactivates several coagulation factors.
Uses & Indications
Dosage
Adults
Initial IV bolus 5000 IU, then continuous IV infusion 1000-2000 IU/hour, adjusted based on aPTT. Subcutaneous dosage varies for prophylaxis, typically 5000 IU 2-3 times daily.
Elderly
Lower doses may be required, close monitoring essential due to increased bleeding risk.
Renal_impairment
No significant dose adjustment needed for renal impairment, but careful monitoring due to potential accumulation.
How to Take
Administer by intravenous (IV) injection or continuous IV infusion, or by deep subcutaneous (SC) injection. Do not administer intramuscularly (IM) due to risk of hematoma.
Mechanism of Action
Heparin exerts its anticoagulant effect by binding to antithrombin III (AT III) and increasing its activity by approximately 1000-fold. This enhanced AT III then inactivates thrombin (Factor IIa) and Factor Xa, preventing the formation of fibrin and subsequently blood clots.

Immediate (IV), 20-60 minutes (SC).
Excretion
Renal excretion (small amount unchanged, majority as inactive metabolites).
Half life
Dose-dependent, 1-2 hours (IV bolus of 100 IU/kg).
Absorption
Poorly absorbed orally, given parenterally. Peak plasma levels minutes after IV, 2-4 hours after SC.
Metabolism
Partially metabolized in the liver by heparinase.
Side Effects
Contraindications

- •History of Heparin-induced thrombocytopenia (HIT)

- •Recent major surgery (especially brain, spinal cord, eye)
- •Bleeding disorders (e.g., hemophilia)
Drug Interactions
NSAIDs
Increased risk of bleeding.
Oral anticoagulants (e.g., warfarin)
Additive anticoagulant effect, requiring careful monitoring when transitioning.
Antiplatelet agents (e.g., aspirin, clopidogrel)
Increased risk of bleeding.
Other anticoagulants (e.g., dabigatran, rivaroxaban)
Significantly increased bleeding risk.
Storage
Store at controlled room temperature (20-25°C), protect from freezing and excessive heat. Do not refrigerate. Once opened, use within 28 days.
Overdose
Overdose of Heparon can lead to severe bleeding. The anticoagulant effect can be rapidly neutralized by the intravenous administration of Protamine Sulfate. Dosage of protamine sulfate depends on the amount of heparin given and time elapsed.
Pregnancy & Lactation
Pregnancy Category C. Heparin does not cross the placenta and is generally considered safe for use during pregnancy when clinically indicated. It is not excreted into breast milk and is considered safe for breastfeeding mothers.
