Description
Overview of the medicine
Hepatab 550 mg Tablet contains Ademetionine, which is a naturally occurring compound in the body. It plays a vital role in various biochemical processes, including methylation, transsulfuration, and aminopropylation, essential for maintaining liver health and detoxification.
Uses & Indications
Dosage
Adults
The usual dosage for intrahepatic cholestasis is 550 mg (one tablet) once or twice daily, preferably between meals. Doses up to 1100 mg daily may be used depending on severity and physician's discretion.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential age-related decline in hepatic and renal function.
Renal_impairment
No specific dosage adjustment is usually required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Hepatab tablets should be swallowed whole with water, preferably between meals to improve absorption. Do not crush or chew the tablet.
Mechanism of Action
Ademetionine acts as a methyl donor in transmethylation reactions, a precursor to glutathione synthesis (a powerful antioxidant), and a polyamine precursor. These actions help to stabilize hepatocyte membranes, promote liver detoxification, reduce oxidative stress, and improve bile flow, thereby protecting liver cells and supporting liver function.
Pharmacokinetics
Onset
Therapeutic effects may take several days to weeks to become apparent, depending on the indication.
Excretion
Primarily excreted via renal pathways as inactive metabolites. A small amount is excreted in feces.
Half life
Approximately 1.5 hours to 2 hours (after IV administration; oral half-life data less defined due to low bioavailability).
Absorption
Oral absorption is variable and low (approx. 5-10%). Bioavailability is significantly higher with parenteral administration. Peak plasma concentrations are reached within 2-6 hours after oral administration.
Metabolism
Extensively metabolized in the liver via transmethylation, transsulfuration, and aminopropylation pathways. Not significantly metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to Ademetionine or any components of the tablet.
- Patients with genetic defects affecting methionine cycle (e.g., homocystinuria, hyperhomocysteinemia due to vitamin B12 deficiency).
- Patients with bipolar disorder (mania/hypomania episodes reported).
Drug Interactions
L-DOPA
Ademetionine may reduce the therapeutic effects of L-DOPA in Parkinson's disease due to methylation processes.
Vitamin B12 and Folate
These vitamins are essential for Ademetionine metabolism; deficiencies can lead to increased homocysteine levels.
Antidepressants (SSRIs, TCAs)
May increase the risk of serotonin syndrome, especially at high doses of Ademetionine.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include gastrointestinal upset. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Ademetionine has been used for intrahepatic cholestasis in pregnancy under medical supervision. However, use during pregnancy and lactation should only be considered if the potential benefit outweighs the potential risk. Consult your doctor.

Lab Monitoring
- Liver function tests (ALT, AST, ALP, Bilirubin) for cholestasis monitoring.
- Homocysteine levels, especially in patients at risk of B12/folate deficiency.
- Monitoring of mood in patients with a history of mood disorders.
Doctor Notes
- Ademetionine is an important adjunctive therapy for chronic liver disease patients with cholestasis.
- Monitor patients with mood disorders carefully due to potential for exacerbation of manic symptoms.
- Advise patients on proper administration (between meals, swallowed whole) for optimal absorption.
Patient Guidelines
- Take the tablet exactly as prescribed by your doctor.
- Inform your doctor about all other medications and supplements you are taking.
- Report any unusual or severe side effects immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Hepatab generally does not affect the ability to drive or operate machinery. However, if you experience dizziness or other side effects that impair your concentration, avoid these activities.
Lifestyle Advice
- Maintain a healthy diet and limit alcohol consumption to support liver health.
- Avoid self-medication and always consult your doctor for any concerns.
