Overview of the medicine
Herculix 2 gm Injection contains Ceftriaxone Sodium, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
Typical adult dosage is 1-2 gm administered once daily (or in two equally divided doses) depending on the type and severity of infection. In severe cases, up to 4 gm may be given daily.
Elderly
No specific dosage adjustment is necessary for elderly patients with normal renal and hepatic function. However, renal function should be monitored.
Renal_impairment
In patients with severe renal impairment (creatinine clearance <10 mL/min) without hemodialysis, the dosage should not exceed 2 gm daily. No adjustment needed for mild to moderate impairment.
How to Take
Herculix 2 gm Injection can be administered by deep intramuscular injection or by slow intravenous infusion (over 30 minutes). It should not be mixed with calcium-containing solutions.
Mechanism of Action
Ceftriaxone is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid, with bactericidal concentrations achieved quickly.
Excretion
Approximately 50-60% is excreted in the urine (primarily unchanged) and 40-50% in the bile as unchanged drug or inactive metabolites.
Half life
Approximately 6-9 hours in adults, allowing for once-daily dosing.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are achieved shortly after IV infusion or 2-3 hours after IM injection.
Metabolism
Minimal hepatic metabolism; about 35-65% is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, any other cephalosporins, or to any of the excipients.
- •Neonates with hyperbilirubinemia, especially premature neonates.
- •Concomitant use with calcium-containing IV solutions in neonates (≤28 days of age).
Drug Interactions
Anticoagulants
May increase the risk of bleeding due to vitamin K antagonism.
Aminoglycosides
May increase nephrotoxicity, monitor renal function.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise using alternative birth control.
Loop diuretics (e.g., Furosemide)
Increased risk of nephrotoxicity.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Reconstituted solution should be used immediately or within specified time if refrigerated.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be given. Ceftriaxone is not effectively removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy should be exercised with caution and only if clearly needed. Ceftriaxone is excreted in low concentrations in breast milk; caution should be exercised when administered to a nursing woman.
