Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat a wide range of bacterial infections such as those of the ear, nose, throat, respiratory tract, skin, and urinary tract. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on the severity and type of infection. For severe infections, up to 500 mg every 8 hours.
Elderly
Dosage generally similar to adults, but renal function should be monitored and dosage adjusted if necessary.
Renal_impairment
Dosage adjustment required. For CrCl < 30 mL/min, dosage intervals may need to be extended or doses reduced.
How to Take
Taken orally with or without food. For best absorption and to reduce gastrointestinal upset, it can be taken with food. Swallow the capsule whole with water.
Mechanism of Action
Amoxicillin is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, leading to impaired peptidoglycan synthesis, resulting in cell lysis and bacterial death.

Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion).
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, stable in the presence of gastric acid. Bioavailability is approximately 70-90%.
Metabolism
Limited hepatic metabolism; approximately 10-25% is metabolized to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to amoxicillin or any other penicillin
- •History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., cephalosporins, carbapenems)
Drug Interactions
Warfarin
May increase the anticoagulant effect, requiring monitoring of INR.
Allopurinol
Increased risk of skin rash.
Methotrexate
May reduce methotrexate excretion, potentially increasing toxicity.
Oral Contraceptives
May decrease efficacy of oral contraceptives (though evidence is weak, advise additional contraception).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, abdominal pain, and rarely, crystalluria leading to renal failure. Treatment is supportive and symptomatic; hemodialysis may be used to remove the drug from circulation in cases of severe overdose or renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Amoxicillin is generally considered safe during pregnancy when clearly needed. It is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential adverse effects (e.g., diarrhea, candidiasis).
