Description
Overview of the medicine
Hydrason 20 mg Tablet is an angiotensin-converting enzyme (ACE) inhibitor used to treat high blood pressure (hypertension) and heart failure. It helps relax blood vessels, lowering blood pressure and making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial dose: 10 mg once daily. Maintenance dose: 20-40 mg once daily, adjusted based on blood pressure response. Max dose: 80 mg/day.
Elderly
Initial dose: 5 mg once daily. Dosage should be adjusted carefully due to potential for decreased renal function.
Renal_impairment
Initial dose: 5 mg once daily for CrCl < 30 mL/min. Adjust dose according to response and creatinine clearance.
How to Take
Take Hydrason 20 mg Tablet orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Vasonapril, an ACE inhibitor, prevents the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to decreased vasoconstriction, reduced aldosterone secretion (leading to less sodium and water retention), and inhibition of bradykinin degradation, collectively resulting in vasodilation and reduced blood pressure.

Primarily excreted unchanged by the kidneys (Vasonaprilat) and via urine (Vasonapril).

Approximately 11 hours (Vasonaprilat)
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 25-30%. Peak plasma concentrations are reached within 1 hour for Vasonapril and 3-4 hours for its active metabolite, Vasonaprilat.
Metabolism
Vasonapril is a prodrug that is extensively hydrolyzed in the liver to its active metabolite, Vasonaprilat.
Side Effects
Contraindications
- History of angioedema related to previous ACE inhibitor treatment
- Hereditary or idiopathic angioedema
- Pregnancy (especially 2nd and 3rd trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
- Hypersensitivity to Vasonapril or any other ACE inhibitor
Drug Interactions
Lithium
Increased serum lithium levels and lithium toxicity.
Aliskiren
Contraindicated in diabetic patients and those with renal impairment.
Diuretics
Increased risk of hypotension.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
Reduced antihypertensive effect and increased risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension (low blood pressure), bradycardia, shock, electrolyte disturbances, and renal failure. Treatment is symptomatic and supportive, including intravenous fluid administration for hypotension and hemodialysis if severe renal failure occurs. Closely monitor blood pressure and serum electrolytes.
Pregnancy & Lactation
Hydrason is contraindicated in pregnancy, especially during the second and third trimesters, due to the risk of fetal injury or death. It is not recommended during breastfeeding, as Vasonaprilat is excreted in human milk.
