Description
Overview of the medicine
Iben 400 mg Tablet contains Ibuprofen, a non-steroidal anti-inflammatory drug (NSAID) used for relieving pain, reducing inflammation, and lowering fever. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
Oral: 200-400 mg every 4-6 hours as needed. Maximum dose: 1200 mg/day for OTC use; up to 2400 mg/day for prescription use.
Elderly
Use lowest effective dose for the shortest duration. Monitor for renal and gastrointestinal adverse effects.
Renal_impairment
Use with caution. Dosage adjustment may be required based on severity of impairment. Avoid in severe renal failure.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal upset. Do not crush or chew the tablet.
Mechanism of Action
Ibuprofen is a non-selective inhibitor of cyclooxygenase (COX-1 and COX-2) enzymes. This inhibition reduces the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Analgesic effect: 30-60 minutes; Antipyretic effect: 1 hour; Anti-inflammatory effect: within 7 days.
Excretion
Excreted rapidly and completely in the urine (approximately 60-90% as metabolites and their conjugates, and 1% as unchanged drug) within 24 hours.
Half life
Approximately 1.8 to 2.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are typically achieved within 1 to 2 hours after oral administration.
Metabolism
Extensively metabolized in the liver (primarily via CYP2C9 and CYP2C8) to two inactive metabolites.

- Coronary artery bypass graft (CABG) surgery
Drug Interactions
SSRIs
Increased risk of gastrointestinal bleeding.
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
ACE inhibitors/Diuretics
Reduced antihypertensive and diuretic effects, increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, lethargy, drowsiness, nystagmus, tinnitus, and headache. More severe cases may involve gastrointestinal bleeding, acute renal failure, coma, metabolic acidosis, and respiratory depression. Management includes gastric lavage, activated charcoal, and supportive treatment.
