Overview of the medicine
Irbesartan is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure and to protect the kidneys from damage due to diabetes.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose is 75 mg once daily. The dose can be increased to 150 mg once daily, and if needed, to a maximum of 300 mg once daily. Diabetic Nephropathy: Initial dose is 75 mg once daily, usually escalated to 300 mg once daily.
Elderly
No initial dosage adjustment is usually necessary for elderly patients.
Renal_impairment
No initial dosage adjustment is necessary for patients with mild to moderate renal impairment. For severe impairment, caution is advised, and close monitoring of renal function and potassium levels is recommended.
How to Take
Ibsan 75 mg Tablet should be taken orally once daily, with or without food. It is recommended to take the tablet at approximately the same time each day.
Mechanism of Action
Irbesartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.

Onset
Antihypertensive effect begins within 1-2 hours, with maximum effect generally achieved within 4-6 hours.
Excretion
Approximately 20% of the administered dose is excreted in the urine (primarily unchanged), and the remainder (80%) is excreted in the feces via biliary excretion.
Half life
The elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 60-80%. Peak plasma concentrations are reached 1.5-2 hours after oral administration.
Metabolism
Minimally metabolized by the liver, primarily via oxidation by CYP2C9. Approximately 20% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to irbesartan or any of the excipients.
- Pregnancy (especially during the second and third trimesters) due to risk of fetal toxicity.
- Concomitant use with aliskiren-containing products in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Diuretics and other antihypertensive agents
May increase the hypotensive effect.
Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)
May reduce the antihypertensive effect and increase the risk of renal function deterioration, including possible acute renal failure.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes
May lead to hyperkalemia.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not use after the expiry date.
