Description
Overview of the medicine
Icosa-1 gm Capsule contains Icosapent Ethyl, a highly purified omega-3 fatty acid (eicosapentaenoic acid ethyl ester). It is primarily used as an adjunct to diet to reduce very high triglyceride levels and as an adjunct to statin therapy to reduce the risk of cardiovascular events in certain patients.
Uses & Indications
Dosage
Adults
2 gm orally twice daily with food (total 4 gm/day).
Elderly
No specific dose adjustment is required based on age alone, but caution should be exercised due to potential for decreased renal function.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment. Data limited for severe renal impairment.
How to Take
Take orally with food. Swallow capsules whole; do not break, crush, dissolve, or chew.
Mechanism of Action
Icosapent ethyl reduces hepatic very low-density lipoprotein triglyceride (VLDL-TG) synthesis and secretion, and enhances triglyceride clearance from VLDL particles. It may also inhibit diacylglycerol acyltransferase, increase beta-oxidation, and reduce circulating fatty acids that are precursors to triglycerides. Additionally, it has anti-inflammatory and anti-platelet effects.
Pharmacokinetics
Onset
Triglyceride-lowering effects typically observed within weeks; cardiovascular benefits take longer (months to years).
Excretion
Excreted primarily as carbon dioxide and water via respiration and urine, with minimal fecal excretion.
Half life
Approximately 89 hours for total EPA after single dose, with steady state reached in about 2 weeks.
Absorption
Readily absorbed after oral administration, peak plasma concentrations typically achieved within 5-9 hours. Absorption is enhanced with a high-fat meal.
Metabolism
Primarily metabolized in the liver by hepatic lipases to eicosapentaenoic acid (EPA) and then through beta-oxidation, similar to dietary fatty acids.
Side Effects
Contraindications
- Known hypersensitivity to icosapent ethyl or any components of the product.
- Known hypersensitivity to fish and shellfish.
Drug Interactions
Anticoagulants/Antiplatelet Agents
Increased risk of bleeding. Monitor patients receiving concurrent therapy with anticoagulants or antiplatelet agents.
Storage
Store at room temperature (20°C to 25°C), away from moisture and light. Do not freeze.
Overdose
Overdose experience is limited. Treatment should be symptomatic and supportive. It is not dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Caution advised during lactation; it is unknown if icosapent ethyl is excreted in human milk.
