Description
Overview of the medicine
Idiopag 50 mg Tablet is a selective immunomodulator used in the treatment of various autoimmune and inflammatory conditions. It works by targeting specific pathways involved in immune responses.
Uses & Indications
Dosage
Adults
50 mg once daily, with or without food. Dosage may be adjusted based on clinical response and tolerability.
Elderly
No specific dose adjustment is required in elderly patients, but monitor for adverse effects.
Renal_impairment
For moderate renal impairment (CrCl 30-60 mL/min), reduce dose to 25 mg once daily. Not recommended for severe impairment.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole; do not crush, chew, or split.
Mechanism of Action
Idiopag selectively inhibits Janus Kinase (JAK) enzymes, particularly JAK1 and JAK2. By inhibiting these enzymes, it modulates cytokine and growth factor signaling, thereby suppressing the inflammatory and immune responses.
Pharmacokinetics
Onset
Clinical effects may be observed within 2-4 weeks of initiation.
Excretion
Excreted predominantly via urine (approximately 70%) and feces (approximately 30%).
Half life
Approximately 8-10 hours, allowing for once or twice daily dosing.
Absorption
Rapidly absorbed, with peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily metabolized by cytochrome P450 (CYP) enzymes, mainly CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Idiopag or any excipients
- •Active serious infections (e.g., tuberculosis, sepsis)
- •Severe hepatic impairment
- •Pregnancy and breastfeeding
Drug Interactions
Strong CYP3A4 Inducers (e.g., Rifampicin)
May decrease Idiopag exposure, potentially reducing efficacy.
Other Immunosuppressants (e.g., Cyclosporine)
Increased risk of infections and other adverse events; generally not recommended.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole)
May increase Idiopag exposure, necessitating dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, no specific antidote is available. Management should be supportive and symptomatic. Hemodialysis is unlikely to be effective.
Pregnancy & Lactation
Pregnancy Category D: May cause fetal harm. Not recommended during pregnancy. Breastfeeding: Excreted in breast milk; advise women not to breastfeed during treatment.
