Description
Overview of the medicine
Iluca-150 mg Capsule contains Pregabalin, an anticonvulsant and neuropathic pain agent. It is used to treat neuropathic pain, fibromyalgia, and as an add-on therapy for partial-onset seizures.
Uses & Indications
Dosage
Adults
For neuropathic pain, initial dose 75 mg twice daily or 50 mg three times daily, adjusted based on response and tolerability, up to a maximum of 600 mg/day. A 150 mg capsule is often taken once or twice daily. For fibromyalgia, start 75 mg BID, can increase to 150 mg BID.
Elderly
Dose adjustment may be necessary in elderly patients due to age-related decrease in renal function. Starting with a lower dose and careful titration is advised.
Renal_impairment
Significant dose reduction is required based on creatinine clearance. For CrCl 30-60 mL/min, max 300 mg/day; for CrCl 15-30 mL/min, max 150 mg/day; for CrCl <15 mL/min, max 75 mg/day or 25-50 mg daily/every other day.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not chew, crush, or open the capsule.
Mechanism of Action
Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby modulating the release of various neurotransmitters including glutamate, norepinephrine, and substance P. This action reduces neuronal excitability.

Onset
Analgesic effects typically begin within 1 week for neuropathic pain. Anxiolytic effects may appear sooner.
Excretion
Primarily excreted unchanged in the urine (approximately 98% of the dose).
Half life
Approximately 6.3 hours, independent of dose.
Absorption
Rapidly absorbed following oral administration with high bioavailability (≥90%). Peak plasma concentrations are reached within approximately 1.5 hours.
Metabolism
Pregabalin undergoes negligible metabolism in humans; less than 0.1% of the dose is excreted as metabolites.
Side Effects
Contraindications
- Hypersensitivity to Pregabalin or any of the excipients in the formulation.
- Severe respiratory depression (due to potential for exacerbation).
Drug Interactions
ACE inhibitors
May increase the risk of angioedema when co-administered with Pregabalin.
CNS depressants (opioids, benzodiazepines, ethanol)
Concomitant use may potentiate CNS depressant effects, leading to increased somnolence and respiratory depression.

Symptoms of overdose include somnolence, confusion, agitation, and, rarely, coma. Treatment is supportive and symptomatic. Hemodialysis may be useful in patients with compromised renal function.

Pregnancy Category C: Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug.
